抗血管内皮生长因子在视网膜疾病中的应用

Treatment of retinal diseases with anti-vascular endothelial growth factor

抗血管内皮生长因子(VEGF)药物的出现及其在视网膜领域的应用,是视网膜疾病的一次革命性举措。一些过去无法治疗或疗效不佳的视网膜疾病,如年龄相关性黄斑变性(AMD)、糖尿病视网膜病变(DR)、视网膜静脉阻塞(RVO)等,在应用抗VEGF药物后使绝大多数患者保存了视力,约1/3病例视力甚至提高。本文叙述视网膜疾病中,根据循证医学及文献复习,提出哪些应是首选使用,哪些可以考虑使用,哪些则是无效的,并简单介绍它们的用法及并发症。

The advent of anti-vascular endothelial growth factor （VEGF）and its administration in retinal vascular diseases such as age-related macular degeneration （AMD） , diabetic retinopathy and retinal vein occlusion etc. are revolutionary events. Those previously considered untreatable diseases, now become manageable with intravitreal injection of anti-VEGF. Clinical results showed not only visual acuity can be preserved in most of the patients, even vision gained in about 1/3 of the cases after treatments. Based on evidence-based medicine as well as literature review, this article described the diseases which will be most benefit from anti-VEGF therapy, and the diseases that may be effective and the diseases probably are ineffective. In addition, the interval of drag application and its adverse side effects were briefly discussed.
