面神经术中实时监护在开放式鼓室成形术中的应用

Application of facial nerve monitoring during tympanoplasty

目的探讨开放式鼓室成形术中进行面神经实时监护对预防神经损伤的应用价值。方法对68例慢性化脓性中耳炎及中耳胆脂瘤患者,使用面神经监护仪对面神经进行术中实时监护,以保护神经,预防神经损伤。术前、术后通过面神经功能分级评估面神经功能。结果 68例鼓室成型术仅1例发生术中面神经损伤,经术中面神经减压及术后药物对症治疗后面瘫完全恢复。结论在开放式鼓室成形术中实行面神经术中监护能有效保护面神经,减少神经损伤。

Objective To evaluate the application value of intraoperative neuromonitoring of facial nerve in tympanoplasty. Methods Sixty-eight patients were enrolled in the study. NIM-Response 2. 0 eleetromyographic monitor system was used during surgery to protect nerve against injury. Facial nerve function was assessed by House- Brackmann grading system before and after the surgery . Results There was only one case occurring nerve injury during tympanoplasty, but with the facial nerve decompression and conservative treatments, no patient had significant facial paralysis. Conclusions Intraoperative neuromonitoring of facial nerve during tympanoplasty is useful for protecting nerve from injury.
