玻璃体切除术联合雷珠单抗玻璃体注射治疗增殖性糖尿病视网膜病变

Effect of Pars plana vitrectomy combined with intravitreal injection of Lucentis in the treatment of PDR

目的探讨玻璃体切除术(PPV)联合雷珠单抗玻璃体注射治疗增殖性糖尿病视网膜病变(PDR)的临床疗效。方法122例PDR患者以数字表法随机分为观察组(n=61)与对照组(n=61),观察组给予PPV联合雷珠单抗玻璃体注射治疗,对照组仅给予PPV治疗,对比两组患者术后视力改善情况、手术时间、术中出血量、眼内压、黄斑中心凹视网膜厚度以及并发症情况。结果观察组患者视力改善率为65.6%,高于对照组的44.3%,差异具有统计学意义(P<0.05);观察组手术时间短于对照组、术中出血量少于对照组,黄斑中心凹视网膜厚度低于对照组,差异均具有统计学意义(P<0.05);主要并发症为无菌性眼内炎、术后再出血、术后感染、再增殖,观察组并发症发生率低于对照组,差异具有统计学意义(P<0.05)。结论PPV联合雷珠单抗玻璃体注射治疗PDR疗效显著,可有效改善患者视力,且术后并发症少,值得推广。

Objective To explore the effect of Pars plana vitrectomy （PPV） combined with intravitreal injection of Lucentis in the treatment of PDR. Methods 122 patients with Prolifertive Dibetic Retinopthy （PDR） were randomly divided into the observation group （n = 61 ） and the control group （n = 61 ） according to the numerical table, the observation group were treated with PPV com- bined with intravitreal injection of Lueentis, the control group were treated with PPV, then the visual acuity improvement, operation time, intraoperative blood loss, intraocular pressure, macular foveal retinal thickness and complication were compared between the two groups. Results The improvement rate of visual acuity was 65.6% in observation group, which was significantly higher than 44.3% in the control group（ P 〈 0.05 ） ; The operation time of the observation group was significantly shorter than that of the control group, the amount of intraoperative bleeding was significantly less than that of the control group, the macular fovea retinal thickness was signifi- candy lower than that of the control group（ P 〈 0.05 ） ; The main complications were aseptic endophthalmitis, postoperative rebleeding, postoperative infection and reproliferation, the incidence rate of the observation group was significantly lower than that of the control group （ P 〈 0.05 ）. Conclusion PPV combined with intravitreal injection of Lucentis has curative effects in the treatment of PDR, it can significantly improve the vision of patients, and has less postoperative complications.
