重庆地区消化内科医师对IBS新诊断标准及FODMAP饮食知晓情况调查

An investigation of the consensus on the new diagnostic criteria of IBS and FODMAP diet among the gastroenterological physicians of Chongqing district

目的了解重庆地区消化内科医师对罗马Ⅳ标准中肠易激综合征(irritable bowel syndrome,IBS)新诊断标准及发酵性寡糖、双糖、单糖及多元醇(fermentable oligo-di-monosaccharides-polyols,FODMAP)饮食的知晓情况。方法对重庆市54家医院的187名消化内科医师进行问卷调查。结果 187名消化内科医师中,有164人(87.7%)了解罗马Ⅳ标准中IBS新诊断标准;其中,四组不同职称医师的了解程度比较,差异有统计学意义(P<0.001),住院医师认知度最低。仅有42人了解FODMAP饮食,占22.5%;住院医师与主任医师相比,差异有统计学意义(P=0.032)。但178人(95.2%)赞同将低FODMAP饮食运用于IBS患者的治疗。结论重庆地区消化内科医师对罗马Ⅳ标准中IBS新诊断标准的知晓度尚可,但对FODMAP饮食了解明显不足,在低年资医师中尤为明显,应加强消化内科医师,特别是低年资医师的知识更新及培训。大多数临床医师支持将低FODMAP运用于IBS的治疗。

Objective To investigate the consensus on the new diagnostic criteria of irritable bowel syndrome（ IBS）in Rome Ⅳ criteria and on the fermentable oligo-di-monosaccharides-polyols（ FODMAP） diet among the gastroenterological physicians of Chongqing district. Methods A questionnaire survey was conducted among 187 gastroenterological physicians in 54 hospitals of Chongqing. Results There were 164 physicians（ 87. 7%） clearly knew the new diagnostic criteria of IBS. There were significant differences among the four groups of professional titles（ P 0. 001）,especially these resident physicians who showed minimal awareness. Only 42 of them（ 22. 5%） showed basic knowledge of FODMAP diet,there were statistical differences between resident physicians and chief physicians（ P = 0. 032）,but 178 of the respondents（ 95. 2%） agreed to use FODMAPS diet to treat IBS. Conclusion The above results indicate a fair knowledge of the Rome Ⅳ criteria but poor knowledge of FODMAP diet management among the gastroenterological physicians of Chongqing district,especially the lower titled physicians. It is necessary to strengthen the training of gastroenterological physicians especially the lower-title physicians to follow the updated guide. Most of the physicians approve the application of FODMAP diet into the treatment of IBS.
