“柔筋开结”手法在冻结期肩关节周围炎治疗中的应用

Application of "Softening Tendon and Untangling Knot" Massage Manipulation in Treatment of Periarthritis of Frozen Shoulder

目的:观察"柔筋开结"推拿手法治疗冻结期肩关节周围炎的临床疗效,探讨本手法在肩关节周围炎治疗中的优势与特点。方法:将冻结期肩关节周围炎患者50例,随机分为治疗组和对照组。治疗组采用"柔筋开结"推拿手法进行治疗,对照组采用双氯酚酸钠缓释片口服进行治疗。两组患者在治疗期间均辅以功能锻炼与康复指导。所有患者在治疗后2周、4周与8周时采用疼痛视觉模拟评分法(visual analogue scale,VAS)、肩关节功能评分对疗效进行评价与分析。结果:所有患者均得到随访,并显示两种治疗均可有效改善患者病情。在治疗初期(2周),两组患者VAS评分与肩关节功能评分结果比较,差异无统计学意义(P>0.05)。随着治疗进展(4周后),治疗组患者VAS与肩关节功能评分结果明显优于对照组,差异有统计学意义(P<0.05)。结论:"柔筋开结"推拿手法在冻结期肩关节周围炎的治疗中疗效肯定且预后良好,在有效缓解疼痛的同时,对关节功能的改善和生存质量的提高较常规治疗具有明显的优势,且无相关不良事件的发生,安全性高且复发率低。

Objective： To evaluate the clinical curative effect of ＂ softening tendon and untangling knot＂ massage manipulation on periarthritis of frozen shoulder,and to analyze and discuss the advantages and characteristics of this method in treatment of periarthritis of shoulder. Methods： Fifty patients with frozen shoulder periarthritis were randomly divided into treatment group and control group. The treatment group was treated with ＂ softening tendon and untangling knot＂ massage. The control group was treated with diclofenac sodium sustained release oral tablets. Two groups of patients,during the treatment,were supplemented with functional exercise and rehabilitation guidance. All patients were evaluated and analyzed in the curative effect by visual analogue scale（ VAS） and shoulder function score at 2 weeks,4 weeks and 8 weeks after treatment. Results： All patients were followed up and showed that both treatments were effective in improving the patient＇ s condition. At the beginning of treatment（ 2 weeks）,VAS score and shoulder function scores of two groups were compared,and the difference was not statistically significant（ P〈0. 05）.With the progress of treatment（ after 4 weeks）,the results of VAS score and shoulder function score of the treatment group were significantly better than that of the control group,and the difference was statistically significant（ P〈0. 05）. Conclusion： ＂ softening tendon and untangling knot＂ massage method is effective in treatment of periarthritis of frozen shoulder. By effectively relieving pain and the improvement of joint function and quality of life,it has obvious advantages over conventional treatment. It has no adverse events with high safety and low recurrence rate.
