免疫球蛋白对轮状病毒性肠炎患儿炎性反应的影响及疗效分析

Effect of Immunoglobulin on Inflammatory Response in Children with Rotavirus Enteritis

目的:观察免疫球蛋白辅助治疗小儿轮状病毒性肠炎的临床效果。方法:选择笔者所在医院儿科轮状病毒性肠炎住院治疗患儿68例,随机分为对照组(给予抗病毒等常规治疗+安慰剂)和观察组(抗病毒等常规治疗+注射用免疫球蛋白静脉滴注),各34例。观察两组患儿治疗前后肿瘤坏死因子(TNF-α)、白细胞介素-6(IL-6)水平及分泌型免疫球蛋白A(SIgA)含量,并比较两组临床疗效。结果:观察组总有效率为79.42%,高于对照组的52.94%,差异有统计学意义(P<0.05)。治疗后,两组患儿TNF-α、IL-6水平均低于于治疗前,且观察组两项指标水平显著低于对照组;两组患儿SIgA分泌量较治疗前均明显升高,且观察组SIgA水平显著高于对照组,差异均有统计学意义(P<0.05)。结论:免疫球蛋白辅助治疗小儿轮状病毒性肠炎效果显著,可缓解患儿肠道炎性反应,延缓病情发展,值得临床推广。

Objective：To investigate the therapeutic effect of Immunoglobulin on pediatric rotavirus enteritis.Method：The 68 children with rotavirus enteritis were selected as the research object,and were randomly divided into control group（conventional antiviral treatment and placebo） and observation group（conventional antiviral treatment and Immunoglobulin）,34 cases in each group.The serum levels of TNF-α,IL-6 changes,SIgA of two groups before and after treatment were observed,and the clinical curative effect were compared.Result：The total efficiency of observation group was 79.42%,which was higher than 52.94% of control group,the difference was statistically significant（P〈0.05）.After treatment,the TNF-and IL-6 levels in the two groups were lower than those before treatment,and the two indicators in the observation group were significantly lower than those in control group（P〈0.05）.The SIgA level in the two groups increased significantly compared with those before treatment,and the SIgA level in observation group was significantly higher than that in the control group,the differences were statistically significant（P〈0.05）.Conclusion：Immunoglobulin can significantly improve the cure rate of pediatric rotavirus enteritis and delay the inflammatory reaction of the intestinal tract,shorten the course of disease.It is worthy of clinical promotion.
