急性脑梗死患者急性期血压变化与早期神经功能恶化的相关性

The Relationship between Blood Pressure Changes and Early Neurological Deterioration in Patients with Acute Ischemic Stroke

目的分析急性脑梗死患者早期血压变化与早期神经功能恶化(END)的相关性。方法研究对象是161例急性脑梗死发作,且在24 h内住院的患者。主要观察脑卒中发作72 h后END的发展,并检测72 h内患者平均(mean)、最高(max)和最低(min)收缩压(SBP)及舒张压(DBP)。结果研究对象平均年龄是67.0±13.5岁,其中59.5%是男性,有29例(18.1%)患者出现END。所有血压变化参数均与END线性相关(P<0.05)。SBPmean、SBPmax、DBPmax和DBPmin与END独立相关。结论急性缺血性脑卒中患者早期血压变化与END发展呈独立和线性相关。

Abatract：Objectives To study association between blood pressure（BP）changes and development of early neurological deterioration（END）in patients with acute ischemic stroke,Methods we studied a consecutive series of patients hospitalized for acute ischemic stroke within 24 h of onset.The primary outcome of interest was the development of END according to predefined criteria within the first 72 h of stroke onset.During this period,the mean,maximum（max）,and minimum（min）values for the SBP and DBP were measured.Results Of the 161 patients enrolled in the study（mean age,67.0±13.5 years;59.5%men）,29（18.1%）developed END.all of the BP variability parameters were linearly associated with END independent of mean BP and potential clinical variables（P0.05.Among the other BP parameters,SBPmean,SBPmax,DBPmax,and DBPmin were independently associated with END.Conclusion BP variability is independently and linearly associated with the development of neurologic deterioration in acute stage of ischemic stroke.
