摘要
以分期为导向的综合诊治仍是胃癌治疗的基石。第8版TNM分期第一次用胃癌的大数据进行了分期的调整,同时随着术前治疗地位的逐渐上升,首次提出的临床分期和术前治疗后病理学分期也带来了新的挑战。伴随着分期调整,术前治疗也出现了新的模式和方案的区别,相关研究可能改变现有治疗格局。手术的细节逐渐得到相关研究结果的确认,但微创手术中的吻合及前哨淋巴结以及晚期胃癌手术的适宜人群还须进一步明确。规范诊治过程中收集的数据及其共享或成为胃癌外科进步的驱动力。
Stage - oriented comprehensive treatment is still the cornerstone of gastric cancer treatment. The eighth edition of TNM staging use big data from IGCA database to adjust the stage for the first time. The first proposed c-staging and yp-staging also bring new challenges. Along with the staging adjustment, new mode and scheme of preoperative treatment are also emerging, and the related research may change the existing pattern. Details of the operation are confirmed following the publication of relevant study, but it is still necessary to further investigate anastomosis of minimally invasive surgery, sentinel lymph node, as well as indication for surgery with advanced stage. Data collection along with standardized treatment, as well as its sharing, my become the driving force for progress.
