镜下缝合止血、超声刀止血对腹腔镜下卵巢囊肿剥除术后短期预后质量的影响

Effect of endoscopic suture hemostasis and ultrasonic scalpel hemostasis on short-term prognosis quality of laparoscopic ovarian cysts

目的探讨镜下缝合止血、超声刀止血对腹腔镜下卵巢囊肿剥除术后短期预后质量的影响。方法选取2015年1月～2017年3月在我院进行治疗的卵巢囊肿患者94例,根据止血方式的不同分为镜下缝合组(47例)和超声刀组(47例),采用酶联免疫吸附测定法(ELISA)检测手术前后患者血清内的促黄体生成素(LH)、卵泡刺激素(FSH)及雌二醇(E2)的水平,并比较手术前后的卵巢间质动脉收缩期峰值流速(PSV)和卵巢基础窦卵泡计数(AFC)。结果手术前,两组患者的LH、FSH及E2的水平相比,差异均无统计学意义(P>0.05);手术后3个月,与超声刀组相比,镜下缝合组患者的LH及FSH的水平显著降低,而E2水平显著升高,差异具有统计学意义(P<0.05);术前两组患者的PSV和AFC相比,差异均无统计学意义(P>0.05);手术后3个月,与超声刀组相比,镜下缝合组患者的PSV和AFC显著提高,差异具有统计学意义(P<0.05);与超声刀组相比,镜下缝合组患者的妊娠时间及妊娠高峰时间均显著缩短,差异具有统计学意义(P<0.05)。结论相比于超声刀止血,镜下缝合止血可显著改善腹腔镜下卵巢囊肿剥除术后患者的短期预后质量。

Objective To investigate the effect of endoscopic suture hemostasis and ultrasonic scalpel hemostasis on short-term prognosis quality of laparoscopic ovarian cysts. Methods Ninety-four patients with ovarian cysts were selected from January 2015 to March 2017 in our hospital, and they were divided into microscopic suture group （47 cases） and ultrasonic scalpel group （47 cases） according to the different hemostatic ways. Serum luteinizing hormone （luteinizing, hormone, LH）, follicle stimulating hormone （follicle stimulating, hormone, FSH） and estradiol （estradiol, E2） level in patient before and after surgery were detected by Enzyme linked immunosorbent assay （enzyme-linked immunosorbent assay, ELISA）. The peak systolic blood flow velocity （PSV） and the ovarian basal sinus follicle count （antral follicle count, AFC） were compared before and after operation. Results Before operation, there was no significant difference in the levels of LH, FSH and E2 between the two groups （P 〉 0.05）; 3 months after operation, compared with the ultrasonic scalpel group, the level of LH and FSH in the suture group decreased significantly, while the E2 level increased significantly, the difference was statistically significant （P 〈 0.05）; There was no significant difference in PSV and AFC between the two groups hefpre operation （P 〉 0.05）; After 3 months of operation, compared with the ultrasonic knife group, the PSV and AFC in the suture group were significantly improved, the difference was statistically significant （P 〈 0.05）; Compared with the ultrasonic knife group, the time of pregnancy and the peak time of pregnancy in the suture group were significantly shortened, and the difference was statistically significant （P 〈 0.05）. Conclusion Compared with the ultrasonic scalpel hemostasis, the endoscopic suture of hemostasis can significantly improve the short-term prognosis quality of the patients after laparoscopic ovarian cystectomy.
