从脑卒中病人个案管理的应用模式、核心人员、实施过程、效果评价及关键进行综述,提出需要尝试借鉴国外的经验和标准探索适合中国道路的脑卒中个案管理实践体系。

This article reviewed case management on stroke patients from application model,core personnel,implementation process,effect evaluation and key measure.It pointed out that through learning from foreign experience and standard,the case management system on stroke patients which suited for situation in China could be built.
