The Effects of Alcohol Ingestion on OSAHS and Relationship between ADH2/ALDH2 Genetic Polymorphisms and Obstructive Sleep Apnea Syndrome

摘要 目的探讨饮酒及乙醇脱氢酶2(ADH2)、乙醛脱氢酶2(ALDH2)基因多态性与阻塞性睡眠呼吸暂停低通气综合征(obstructive sleepapnea hypopnea syndrome,OSAHS)的关系。方法以2014年2月—2017年2月因打鼾在我院就诊并行多导睡眠呼吸监测的321例男性为研究对象,经多导睡眠呼吸监测后分为OSAHS组208例和非OSAHS组113例。回顾性统计研究对象一般资料;采用聚合酶链式反应-连接酶检测反应技术检测ADH2 Arg47His和ALDH2 Glu487Lys基因型,应用logistic回归分析评价OSAHS发病危险因素,并评价基因多态性与OSAHS病变程度的相关性。结果 OSAHS组颈围、腰围大于非OSAHS组,鼾龄长于非OSAHS组,体重指数、高血压病患病率、糖尿病患病率、平均收缩压、平均舒张压均高于非OSAHS组,差异均有统计学意义(P<0.05或P<0.01)。两组基因型和等位基因分布频率比较差异无统计学意义(χ~2=1.23,χ~2=2.37,P均>0.05);大量饮酒是OSAHS发病的危险因素(OR=1.022,95%CI=0.990,0.990);与携带ADH2(A/A)或ALDH2(G/G)基因型者比较,其他基因型者患OSAHS的危险性未增加(P>0.05)。结论大量饮酒与OSAHS的发病相关;ADH2、ALDH2基因多态性与OSAHS易感性无关;OSAHS严重程度与ADH2及ALDH2基因多态性无关联。 Objective To evaluate the effects of alcohol ingestion on obstructive sleep apnea hypopnea syndrome (OSAHS) and the relationship between functional ADH2 Arg47His and ALDH2 Glu487Lys polymorphisms and OSAHS. Methods The data were collected from a chart review of patients with sleep snoring, all patients who have been tested for PSG in our center from February 2014 to February 2017 were surveyed. A total of 321 patients were excluded, the recruited subjects were divided into OSAHS ( n = 208 ) and non-OSAHS ( n= 113 ) groups. Information about drinking condition was collected. The polymorphisms of ADH2 Arg47His and ALDH2 Glu487Lys were detected by PCR-LDR methods. Unconditional logistic regression analysis was used to adjust the potential confounding factors and to assess the independent effect of each contributing factor on the presence of OSAHS. Results BMI, neck circumference, abdominal circumference and snore age were statistically significant difference respectively between OSAHS group and non-OSAHS group (P 〈0. 05 or P 〈0. 01) . The frequency of ADH2 genotype AA/( A/G + G/G) and ALDH2 genotype GG/(G/A + A/A) in OSAHS group had no statistical differences comparing with the control group = 1. 23, = 2. 37, F 〉 0. 05). Compared with ADH2 genotype A/A andALDH2 genotype G/G, other genotypes did not increase the risk of developing the OSAHS. Conclusion Drinking is a risk factor for OSAHS, ADH2 Arg47His and ALDH2 Glu487Lys genetic polymorphisms had no association with OSAHS and the se-verity of OSAHS.
