摘要
目的观察分析冠心病合并糖尿病患者单核细胞/高密度脂蛋白(HDL)比率(MHR值)变化及其与冠状动脉狭窄程度的相关性。方法选取2015年5月至2017年4月郑州大学第一附属医院收治的219例冠心病患者,根据是否合并糖尿病将其分为单纯冠心病组117例、冠心病合并糖尿病组102例,另选取同期无冠心病、糖尿病患者89例作为对照组。MHR值通过单核细胞计数除以HDL计数计算得到,比较各组间MHR值。采用Gensini评分系统测定冠状动脉狭窄程度,比较各组间Gensini评分,并与MHR值进行相关性分析。结果冠心病合并糖尿病组MHR值和Gensini评分均高于单纯冠心病组[(0.537±0.193)分比(0.470±0.165)分、(42.7±31.8)分比(35.4±26.4)分],且单纯冠心病组MHR值和Gensini评分均高于对照组[(0.470±0.165)分比(0.416±0.164)分、(35.4±26.4)分比(3.6±3.8)分],差异有统计学意义(P均<0.01)。经直线相关分析:冠心病合并糖尿病组血清MHR值与Gensini评分呈正相关(r=0.185,P<0.05)。结论冠心病合并糖尿病患者血清MHR值升高,且MHR值越高,冠状动脉狭窄程度越严重,MHR值可作为冠状动脉狭窄程度评估的参考指标。
Objective To observe the changes of monocyte / HDL ratio in patients with coronary heart disease complicated with diabetes mellitns and its relationship with the degree of coronary artery stenosis. Methods Two hundred and nineteen cases of coronary heart disease were admitted to the First Affiliated Hospital of Zhengzhou University from May of 2015 to April of 2017. According to whether the merger of diabetes, they were divided into simple coronary heart disease group with one hundred and seventeen cases, and coronary heart disease complicated with diabetes group with one hundred and two cases. Eighty nine patients without coronary heart disease or diabetes at the same period were selected as control group. The MHR values were calculated by dividing the monocyte count by the HDL count and comparing the MHR values among the groups. The degree of coronary artery stenosis was measured by Gensini scoring system. The Gensini score was compared among three groups and the correlation was analyzed with MHR. Results The MHR and Gensini scores of coronary heart disease patients with diabetes mellitus were significantly higher than those of coronary heart disease patients. The MHR and Gensini scores of coronary heart disease patients were significantly higher than those of controls (P 〈 0. 01 ). The linear correlation analysis: coronary heart disease with diabetes mellitus serum MHR value and Gensini score was positively correlated ( r = 0. 185, P 〈 0. 05 ). Conclusion The serum MHR of patients with coronary heart disease complicated with diabetes mellitus is increased, and tile higher the MHR value is, the more serious the coronary artery stenosis is, which can he used as a reference index to evaluate the degree of coronary artery stenosis.
