用胰岛素注射器向玻璃体内注药安全性的观察

Observation on the safety of application of insulin syringe in the intravitreal injection

摘要 目的观察用胰岛素注射器向玻璃体内注药的安全性。方法选取黄斑水肿82例(82眼),用胰岛素注射器向玻璃体内注射2mg曲安奈德(0.05m1),分别于注射后30min、1、3、7及30d行眼压检查及间接检眼镜检查。结果82眼中,注射后30min眼压高于正常者5眼,占6.10%。注射后1、3、7及30d,眼压均正常。注射部位结膜下出血范围:直径〉10mm者5眼,未超过1个象限;直径〈10mm者7眼,另70眼仅注射部位结膜充血。随访至注射后3个月,3眼原有的晶状体浑浊加重,但不排除与年龄相关性白内障有关。本组均未出现眼内炎、玻璃体积血或视网膜脱离等严重并发症。结论胰岛素注射器用于玻璃体内注药,损伤较小,是一种安全的方式。 Objective To observe the safety of application of insulin syringe in intravitreal injection. Methods The data of 82 eyes of 82 patients with macular edema were enrolled and all cases received intravitreal injection of triamcinolone acetonide (TA) (2mg, 0.05ml ) using insulin syringe. Intraocular pressure measuring and indirect ophthalmoscopy were performed at 30 rain, 1,3, 7, and 30 days after injection. Results Of the 82 eyes, the IOP were increased in 5 eyes ( 6. 10% ) at 30 min after injection. The intraocular pressure at 1, 3, 7 and 30 days after injection were normal. The scope of subconjunctival hemorrhage was 〉 10mm in 5 eyes, was not in excess of 1 quadrant; the bleeding range diameter 〈 10mm was in 7 eyes, and conjunctival congestion of the injection site occurred in 70 eyes. Lens opacity of 3 eyes aggravated at 3 months after injection, not exclude the connection with the age-related cataract. No endophthalmitis, vitreous hemorrhage, or retinal detachment happened. Conclusion Application of insulin syringe in intravitreal injection is a safe method with less damage.
