肛瘘MRI检查CAIPIRINHA-VIBE序列代替SE-T1WI序列的可行性分析 被引量:10

The Feasibility Analysis of CAIPIRINHA-VIBE Sequence Replacing SE-T1WI Sequence in the MR Diagnosis of Anal Fistula

摘要 目的研究CAIPIRINHA-VIBE序列在肛瘘MRI检查中代替SE-T1WI序列的可行性。方法回顾性分析经手术证实的42例肛瘘患者术前磁共振CAIPIRINHA-VIBE与SE-T1WI序列图像,以手术结果为参照,比较两者在内口、瘘管及外口的显示率差异;比较两序列轴位图像增强前后同一瘘管的信号强度变化百分比(%SI(change))。结果 CAIPIRINHA-VIBE和SE-T1WI增强序列在内口、瘘管及外口的显示率分别为83.0%、92.6%、89.1%和59.6%、96.1%、93.5%,在内口显示率方面CAIPIRINHA-VIBE序列明显优于SET1WI序列,差异有统计学意义(χ~2=6.287,P〈0.05),两者在瘘管和外口显示率方面无统计学差异(校正χ^2=0.177和0.137,P〉0.05);两序列轴位图像增强前后同一瘘管的信号变化幅值(%SI(change))分别为1.69±0.109和1.74±0.136,两者比较无统计学差异(t=0.320,P〉0.05)。结论 CAIPIRINHA-VIBE在临床工作中可以代替SE-T1WI作为肛瘘MRI常规检查序列。 Objective To discuss the feasibility of the SE-T1WI sequence being replaced by CAIPIRINHA-VIBE sequence in the MR diagnosis of anal fistula. Methods The Magnetic resonance images of 42 patients with surgically proved anal fistula were retrospectively analyzed . The CAIPIRINHA-VIBE and SE-T1WI enhancement sequences were obtained. Statistical analysis of the display rate of the two sequences in the inner orifice ,fistula and extrinsic orifice was done. Compare the percent change in signal intensity ( % SIchonse ) in the same fistula before and after image enhancement with two sequential axial images. Results The display rate of the inner orifice, fistula and extrinsic orifice in the CAIPIRINHA- VIBE and the SE-T1WI enhancement sequence were respectively 83.0% ,92.6% ,89.1% -1 59.6% ,96.1% ,93.5%. Inthe display rate of the inner orifice,the CAIPIRINHA-VIBE sequence is superior to the SE-T1WI sequence, the discrepancy has statistical significance( P 〈 0.05 ). In the display rate of fistula and extrinsic orifice, there was no statistical significance (P 〉 0. 05 ). The two sequence axial images before and after enhancement of the same fistula signal intensity percentage change (% SIchange.) were 1.69 ±0. 109 and 1.74 ±0. 136,there was no statistical diffcrenee(t =0. 320,P 〉0.05). Conclusion CAIP1RINHA-VIBE can replace SE-T1WI as a routine sequence in the MR diagnosis of anal fistula.
