社区宣教糖尿病健康管理对患者血糖及相关并发症发生的影响

Effect of Diabetes Health Management on Blood Glucose and Related Complications based on Community Education

目的探讨基于社区宣教糖尿病健康管理在社区2型糖尿病患者中实施对患者血糖和糖尿病并发症发生影响。方法选取2015年3月—2016年6月本社区登记2型糖尿病患者74例随机分为管理组(基于社区宣教糖尿病健康管理,37例)和随访组(常规随访37例),比较两组患者干预前与干预1年后空腹血糖(FGP)、餐后2h血糖(2hFGP)、糖化血红蛋白(HbA1c)水平及糖尿病相关并发症发生情况。结果干预前,两组患者FGP、2hFGP、HbA1c水平无统计学意义(P>0.05);干预1年后,管理组患者FGP、2hFGP、HbA1c水平均低于随访组(P<0.05)。管理组患者干预1年期间,大血管并发症、糖尿病眼底病变、糖尿病周围神经病变发生率均低于随访组(P<0.05)。结论基于社区宣教糖尿病健康管理在2型糖尿病患者中实施可协助控制患者血糖水平,降低糖尿病相关并发症发生率。

Objective To explore the effect of diabetes health management on blood glucose and complications of diabetes based on community education implemented among patients with type 2 diabetes. Methods 74 cases of type 2 diabetes patients registered in the community during March 2015 to June 2016 were randomly assigned into management groupand control group equally: The former group was treated withdiabetes health management based on community education and the latter only with routine follow-up. We compared the two groups in respect 5 of fasting blood glucose( FGP),2 h postprandial blood glucose( 2 hFGP),glycosylated hemoglobin( HbA1c) level and diabetes-related complications before and after one-year intervention. Results Before intervention,there were not statistically significant differences in the levels of FGP,2hFGP and HbA1c in the two groups( P > 0. 05). After one-year intervention,the levels of FGP,2 hFGP and HbA1c in the management group were significantly lower than that in the follow-up group( P < 0. 05). During one-year intervention,the incidences of macrovascular complications,diabetic retinopathy and diabetic peripheral neuropathy in the management group were significantly lowered as compared to the follow-up group( P < 0. 05). Conclusion Diabetes health management based on community education implemented among patients with type 2 diabetes can help control blood glucose level and reduce the incidence of diabetes-related complications.
