“T”形劈冠法在下颌中位近中水平阻生智牙拔除术中的应用

Application of “T”-shaped Splitting Crown Method in the Mandibular in Nearly Horizontal Maxillary Wisdom Tooth

目的分析下颌中位近中水平阻生智牙拔除术中应用"T"形劈冠法的临床效果。方法方便选取该院2010年12月—2016年12月收治的101例下颌中位近中水平阻生智牙拔除术治疗患者,所有患者术中采用"T"形劈冠法,回顾性分析该组患者拔牙时间、术后并发症发生率等情况。结果该组患者拔牙时间7~36 min左右,平均手术时间(15.7±2.2)min。9例患者拔牙根的过程中导致舌侧骨板折裂,复位后保留。术后暂时性下唇麻木2例,10例患者頬部明显肿胀,2例患者出现轻度干槽症,4例患者开口轻度受限,10例患者术区疼痛。结论下颌中位近中水平阻生智牙拔除术中应用"T"形劈冠法的效果良好,具有创伤小、拔牙时间短、术后恢复快等优点,可在临床广泛推广应用。

Objective This paper tries to analyze the application of ＂T＂-shaped splicing method in the treatment of mandibular median proximal level. Methods A total of 101 patients with mandibular midbrain in the middle from December2010 to December 2016 in this hospital were convenient selected and were treated with ＂T＂-shape splicing method, and tooth extraction time, the incidence of postoperative complications and so on were retrospectively analyzed. Results The tooth extraction time was about 7~36 min, average operation time was（15.7±2.2）min. 9 patients in the process of root pulling out caused lingual bone plate fracture, retained after reset. Postoperative temporary lower lip numbness in 2 cases, 10 cases of patients with markedly spondylolysis swollen, 2 cases of patients with mild dry chamfer disease, 4 cases of patients with limited open slightly, 10 patients of operation area pain. Conclusion The effect of ＂T＂-shaped chisel method is good in the middle of the mandibular midbrain, and it has the advantages of small trauma, short tooth extraction time and quick recovery. It can be widely used in clinical practice.
