特异性梅毒螺旋体确认试验与梅毒快速血浆反应素试验在梅毒检验的比较

Comparison of Specific Treponema Pallidum Confirmatory Test with Rapid Treponema Pallidum Antibody Test in Syphilis

目的对比分析梅毒检验中采用特异性梅毒螺旋体确认试验以及梅毒快速血浆反应素试验(RPR)的诊断效果。方法方便选取该院皮肤科2016年10月—2017年8月收治的80例早期梅毒患者,按照不同的检验方法随机分为观察组(80例,实施特异性梅毒螺旋体确认试验,一般采用明胶颗粒凝集试验(TPPA)进行试验)和对照组(80例,实施RPR),对比两组阳性检出率及其与病程的关系等情况。结果该次80例患者临床I期49例,II期26例,潜伏期5例,两组II期、潜伏期阳性检出率均达到100.0%,观察组TPPA检验患者的I期阳性检出率89.8%(44/49)显著高于对照组67.3%(33/49)。两组>10 d患者I期阳性检出率均高达100.0%,观察组TPPA检验患者1~4 d阳性检出率(66.7%)、5~10 d阳性检出率(83.3%)显著高于对照组33.3%、38.9%。结论 TPPA用于梅毒检验的效果优于RPR,可有效提高早期阳性检出率,尽早发现,可作为梅毒确认试验方法。

Objective This paper tries to compare the diagnostic efficacy of specific treponema pallidum test and treponema pallidum rapid plasma test（RPR） in syphilis test. Methods 80 cases of early syphilis patients in this hospital from October2016 to August 2017 in dermatology in accordance with different test methods were randomly divided into the observation group（80 cases, the implementation of specific treponemal test confirmed by gelatin particle agglutination test,（TPPA） test）and the control group（80 cases, RPR）, the positive rate and its relationship with disease of the two groups were compared.Results Among the 80 cases of patients, 49 cases with clinical stage I, 26 cases with stage II, 5 cases of II latency, the positive rate reached 100.0%, the observation group TPPA test in patients with stage I positive rate reached 89.8%（44/49）,significantly higher than the control group of 67.3%（33/49）. The positive rate of more than 10 d in two groups of patients with I was as high as 100.0%. The positive rate of one to four days detection of 66.7% and the positive rate of 5 to 10 d was 83.3% in the observation group and TPPA group, significantly higher than those in the control group of 33.3% and38.9%. Conclusion TPPA is better than RPR in the detection of syphilis. It can effectively improve the positive rate of early detection, and can be used as a test method for syphilis detection as soon as possible.
