ABO血型对原发性非肌层浸润性膀胱癌预后预测的价值

Value of ABO blood group on prognosis of primary non-muscle-invasive bladder cancer

目的探讨ABO血型与原发性非肌层浸润性膀胱癌预后之间的关系。方法回顾性分析2005年1月至2015年1月我院收治的713例原发性非肌层浸润性膀胱癌患者的临床资料,所有患者均在我院接受了经尿道膀胱肿瘤电切术+术后丝裂霉素或盐酸吡柔比星膀胱灌注治疗。统计患者ABO各血型的数量,单变量和多因素竞争风险回归模型分析疾病的复发、进展情况。平均随访时间为63.7个月。结果 713例患者中最终521例纳入分析研究,其中A型血患者241例(46.1%),B型血患者66例(12.7%),O型血患者178例(34.2%),AB型血患者36例(6.9%)。单因素和多因素分析结果显示,A型血患者恶化复发的发生率和进展速度较O型(P=0.046 9、P=0.017 5)、B型(P=0.003 0、P=0.000 4)及AB型血(P=0.014 6、P=0.000 2)患者更高、更快。结论 ABO血型可以影响原发性非肌层浸润性膀胱癌患者的预后,A型血患者具有最高的肿瘤疾病复发和进展率,ABO血型可以成为预测原发性非肌层浸润性膀胱癌复发和进展的一个理想的辅助标志物。

Objective To explore the relationship between ABO blood group and the prognosis of primary non-muscle-invasive bladder cancer. Methods A retrospective study was conducted in713 patients with primary non-muscle-invasive bladder cancer between January 2005 and January2015.Transurethral resection of the bladder tumor was performed in our hospital,and mitomycin and pirarubicin hydrochloride were used in intravesical chemotherapy.Statistics ABO blood group in each blood group number,univariate and multivariate competitive risk regression models were analyzed for disease recurrence and progression.The mean follow-up period was 63.7 months. Results There were 241 patients（46.1%）,66 patients（12.7%）,178（34.2%）and 36（6.9%）with type A,B,O,AB in the ABO blood group,respectively.Univariate and multivariate analysis showed patients with A blood group were associated with higher recurrence and progression rate compared with other three blood groups（O blood group P=0.046 9,P=0.017 5;B blood group P=0.003 0,P=0.000 4;AB blood group P=0.014 6,P=0.000 2）. Conclusions ABO blood group can affect the prognosis of patients with primary non-muscle-invasive bladder cancer.The patients with type A blood group show the highest recurrence and progression rate of tumor disease.ABO blood type can be an ideal secondary marker for predicting the recurrence and progression of primary non-muscle-invasive bladder cancer.
