钡餐造影与胃镜检查对诊断消化性溃疡的价值对比分析

Comparative Analysis of the Value of Barium Meal and Gastroscopy in the Diagnosis of Peptic Ulcer

目的探讨并对比钡餐造影与胃镜检查对诊断消化性溃疡的价值。方法随机选取笔者所在医院2015年6月—2017年6月期间的100例怀疑为消化性溃疡的患者,应用双盲的方法 ,对100例患者分别进行钡餐检查及胃镜检查,两者相隔5 d,检查之前禁食12 h,对100例患者的钡餐检查结果及胃镜检查结果进行统计分析。结果钡餐检查对单纯胃溃疡、单纯十二指肠溃疡、胃溃疡合并十二指肠溃疡、溃疡伴有幽门梗阻、溃疡合并出血的诊断率分别为33%、29%、22%、7%、9%,胃镜检查对单纯胃溃疡、单纯十二指肠溃疡、胃溃疡合并十二指肠溃疡、溃疡伴有幽门梗阻、溃疡合并出血的诊断率分别为34%、31%、20%、7%、8%,两种检查方法患者消化性溃疡疾病的诊断率均差异无统计学意义(P>0.05)。钡餐检查的假阳性例数3例,分别为胃溃疡合并十二指肠溃疡2例,溃疡合并出血1例,假阳性率为3%,假阴性例数3例,分别为单纯胃溃疡1例,单纯十二指肠溃疡2例,假阴性率3%。钡餐检查总满意率98%,显著高于胃镜检查80%,差异有统计学意义(P<0.05)。结论对于消化性溃疡患者的诊断方式,钡餐检查与胃镜检查具有相似的诊断率,钡餐检查是一种比较快捷、比较方便的检查方式,能够被多数患者所接受,是临床上诊断消化性溃疡的首选辅助检查手段,具有较高的诊断价值。

Objective This paper tries to investigate and compare the value of barium meal and gastroscopy in the diagnosis of peptic ulcer.Methods 100 cases of suspected peptic ulcer patients in this hospital from June 2015 to June 2017 were random selected,double-blind method was used to examine these patients with the barium meal and gastroscopy,with 5 days separation,fasting for 12 hours before examination,and then the inspection results of 100 cases of patients with barium meal examination and gastroscopy were statistically analyzed.Results The diagnosis rate of barium meal examination on simple gastric ulcer,duodenal ulcer,gastric ulcer with simple duodenal ulcer,ulcer with pyloric obstruction,ulcer bleeding were 33%,29%,22%,7%,9%; and those in the gastroscopy were 34%,31%,20%,7%,8%,so there were no statistically significant differences in the diagnosis rate between the two kinds of diagnostic examination methods in patients with peptic ulcer（P〉0.05）.There were 3 cases of false positive cases of barium meal examination,including 2 cases of gastric ulcer complicated with duodenal ulcer,1 case of ulcer hemorrhage and 3% of false positive rate; 3 cases of false negative cases,1 case of simple gastric ulcer,2 cases in simple duodenal ulcer,false negative rate of 3%.The total satisfaction rate of barium meal examination was 98%,which was significantly higher than that of gastroscopy of 80%,and there was statistical difference（P 0.05）.Conclusion For the diagnosis of patients with peptic ulcer,barium meal examination and gastroscopy have similar diagnostic rate,while the barium meal examination is a more efficient and more convenient way of checking,which can be accepted by the majority of patients,can be the first examination method in the clinical diagnosis of peptic ulcer,which has high diagnostic value.
