阴道镜诊断宫颈上皮内瘤变和宫颈癌的临床价值

Clinical Significance of Colposcopy in the Diagnosis of Cervical Intraepithelial Neoplasia and Cervical Cancer

目的探讨阴道镜在宫颈上皮内瘤变与宫颈癌诊断中的临床价值。方法方便选择2014年4月—2016年4月该院收治的580例患者作为研究对象,对其进行阴道镜检查诊断,并将其检查结果与病理组织学检查结果对比。结果将组织病理学的检查结果作为标准,其中CINⅠ患者为170例,CINⅡ患者为73例,CINⅢ患者为22例,宫颈癌患者为7例。阴道镜检查CINⅠ的诊断符合率91.86%,CINⅡ的诊断符合率为91.78%,CINⅢ的诊断符合率为95.45%,宫颈癌的诊断符合率为100.00%;与组织病理学检查结果相比,改良版Reid评分诊断CINⅠ的符合率为64.13%,CINⅡ的符合率为90.77%,CINⅢ的符合率为80.95%。结论对宫颈上皮内瘤变与宫颈癌患者进行阴道镜检查,具有较高的诊断价值,对癌前病变的诊断性较高,具有较高的临床应用价值。

Objective This paper tries to investigate the clinical value of colposcopy in the diagnosis of cervical intraepithelial neoplasia and cervical cancer.Methods 580 patients admitted in this hospital from April 1 st,2014 to April 30 th,2016 were convenient selected as the subjects,and were diagnosed by colposcopy.The results were compared with the histopathological findings.Results Histopathological examination was used as the standard,the 170 cases were CIN Ⅰ,73 cases were CINⅡ,22 cases were CIN Ⅲ and 7 cases were cervical cancer.The coincidence rate of CINⅠ was 91.86%,the coincidence rate of CIN Ⅱ was 91.78%,the coincidence rate of CIN Ⅲ was 95.45%,and the coincidence rate of cervical cancer was 100.0%.Compared with histopathological examination,the coincidence rate of CIN Ⅰby Reid was 64.13%,the coincidence rate of CIN Ⅱ was 90.77%,the coincidence rate of CIN Ⅲ was 80.95%.Conclusion The application of colposcopy on cervical colorectal neoplasia and cervical cancer patients has a high diagnostic value,and the diagnosis of precancerous lesions is high,with a high clinical value.
