面部烧伤后瘢痕行扩张胸三角穿支皮瓣修复的护理体会

Nursing experience of dilated thoracic triangle perforator flap for scar after facial burn

目的分析和总结面部烧伤后瘢痕行扩张胸三角穿支皮瓣修复的护理经验。方法选择于我院行扩张胸三角穿支皮瓣修复术的面部烧伤后瘢痕患者36例,总结扩张器置入前至断蒂术后这一段时间的护理特点与要点。心理护理贯穿整个治疗过程。结果所有患者皮瓣均成活良好,术后随访6个月~2年,患者面部轮廓自然,色泽与邻近皮肤相近,无臃肿觉,柔软度好。患者自觉痛觉、温觉、触觉与对侧稍有差别,但并不明显,对术后效果均较满意。结论心理护理贯穿治疗始终;扩张器置入后及注水期应做好皮瓣护理、疼痛护理,严格无菌操作;皮瓣切取转移术后增强皮瓣管理,注重体位宣教。断蒂术前进行逐步断蒂训练。

Objective To analyze and summarize the nursing experience of the expansion of the expanded thoracic triangle perforator fap for the scar after facial burn. Methods Selected 36 patients with facial burn scar after thoraco delta perforator fap repair in our hospital. Psychological care runs through the whole process of treatment. Results All the faps survived well. After 6 months to 2 years follow-up, the patients had natural contour and similar color to the adjacent skin. They had no swelling and soft feeling. The patient＇s conscious pain, temperature sense, tactile and contralateral diference were slightly diferent, but they were not obvious, and all the results were satisfactory. Conclusion Psychological nursing goes through the treatment all the time. Skin fap nursing, pain nursing and strict aseptic operation should be done after dilator implantation and water injection. After skin fap cutting and transferring, fap management should be strengthened and physical education should be emphasized. The pedicle training was carried out gradually before the pedicle of the pedicle.
