摘要 目的探讨同型半胱氨酸(Hcy)、缺血修饰白蛋白(IMA)、髓过氧化物酶(MPO)、超敏C反应蛋白(hsCRP)与脑血管病患者病变程度的相关性。方法回顾性选取2015年12月至2017年4月收治的96例缺血性脑血管病患者,根据脑血管造影结果分为31例颅内动脉轻度狭窄组、32例中度狭窄组、33例重度狭窄组,另外选取22例颅内动脉无狭窄者作为对照。检测各受试者Hcy、IMA、MPO、hs-CRP水平,对比分析不同病变程度组患者指标间差异,Hcy、IMA、MPO、hs-CRP水平与脑血管病变程度相关性。结果与颅内动脉无狭窄组比较,颅内动脉狭窄组患者血清Hcy、IMA、MPO、hs-CRP水平明显升高(P<0.05),随狭窄程度增加Hcy、IMA、MPO、hs-CRP水平升高(P<0.05),Hcy、IMA、MPO、hs-CRP与颅内动脉病变程度呈正相关(r=0.33,0.32,0.30,0.37,P<0.05);多因素Logistic回归分析显示,年龄、血脂、IMA、Hcy等因素是影响患者颅内血管狭窄程度的独立危险因素(P<0.05)。结论缺血性脑血管病患者血清Hcy、IMA、MPO、hs-CRP水平随脑血管造影显示狭窄程度增加而升高,可能是颅内血管动脉粥样硬化性狭窄病变形成的危险因素。 Objective To explore the relationship between Hcy,IMA,MPO,hs-CRP and degree of disease in patients with cerebrovascular disease. Methods A total of 96 patients with cerebrovascular disease from December 2015 to April 2017 in our hospital were selected. Patients were divided into mild stenosis group with 31 patients,moderate stenosis group with 32 patients,and severe stenosis group with 33 patients.The levels of Hcy,IMA,MPO and hs-CRP were measured. 22 subjects with normal intracranial arteries were selected as control group. The levels of Hcy,IMA,MPO and hs-CRP were compared. The relevance between Hcy,IMA,MPO,hs-CRP and degree of disease was analyzed.Results The levels of Hcy,IMA,MPO and hs-CRP in serum from patients with intracranial arterial stenosis were significantly higher than those in the normal group( P 〈 0. 05). The levels of Hcy,IMA,MPO and hs-CRP were increased with arterial stenosis level( P 〈 0. 05). There was positive correlation between Hcy,IMA,MPO,hs-CRP and intracranial arterial disease( r = 0. 33,0. 32,0. 30,0. 37,P 〈 0. 05). Multivariate logistic regression analysis showed that age,IMA,Hcy and other factors were risk factors affecting the degree of cerebrovascular stenosis in patients( P 〈 0. 05). Conclusion The level of serum Hcy,IMA,MPO and hs-CRP in patients with ischemic cerebrovascular disease increase with arterial stenosis level,which may be risk factors for the formation of cerebrovascular atherosclerotic stenosis.
