摘要
目的探讨胆囊癌、胆囊腺瘤性息肉和慢性胆囊炎组织中p53、β-catenin基因的表达及临床意义。方法选取2012年5月到2017年5月收治的60例胆囊癌患者,20例胆囊腺瘤性息肉患者,20例慢性胆囊炎患者,运用免疫组织化学SP法检测三组患者胆囊组织中p53、β-catenin基因的表达和相关性及其与临床病理特征之间的关系。结果胆囊癌组胆囊组织中p53、β-catenin基因的表达阳性率61.7%(37/60)、68.3%(41/60)显著高于胆囊腺瘤性息肉组35.0%(7/20)、40.0%(8/20)和慢性胆囊炎组10.0%(2/20)、5.0%(1/20)(P<0.05),而胆囊腺瘤性息肉组胆囊组织中p53、β-catenin基因的表达阳性率又显著高于慢性胆囊炎组(P<0.05);胆囊癌组织中p53、β-catenin基因的表达与肿瘤分化程度、Nevin分期、侵犯周围组织均呈显著的正相关关系(P<0.05)。结论胆囊癌组织中p53、β-catenin基因的表达较高,与肿瘤分化程度、Nevin分期、侵犯周围组织均呈显著的正相关关系。检测p53、β-catenin基因表达水平可为胆囊癌患者的治疗和预后的判断提供参考。
Objective To investigate the expressions and clinical significances of p53,β-catenin genes in gallbladder carcinoma,adenomatous polyp of gallbladder and chronic cholecystitis tissues. Methods 60 cases of patients with gallbladder cancer,20 cases of patients were gallbladder adenomatous polyp,20 cases of patients with chronic cholecystitis in our hospital from May 2012 to May 2017 were selected. The p53 and β-catenin genes expressions of the three groups and their corrections with clinicopathological features were detected by immunohistochemical SP method. Results The positive expression rates of p53,β-catenin genes in gallbladder tissue of the gallbladder carcinoma group 61. 7%( 37/60),68. 3%( 41/60) were significantly higher than the gallbladder adenomatous polyp group,chronic cholecystitis group 35. 0%( 7/20),40. 0%( 8/20) vs. 10. 0%( 2/20),5. 0%( 1/20)( P 〈 0. 05). The positive expression rates of p53,β-catenin genes in gallbladder tissue of the gallbladder adenomatous polyps group were significantly higher than the chronic cholecystitis group( P 〈 0. 05). The p53 and β-catenin genes expressions in gallbladder carcinoma had differentiation,with Nevin stages,invasion of surrounding had significant positive correlations( P 〈 0. 05). Conclusion Compared with gallbladder adenomatous polyps and chronic cholecystitis,the expressions of p53,β-catenin genes in gallbladder carcinoma tissue are higher,and it can provide effective basis for the prognosis of gallbladder cancer patients.
