经皮椎间孔镜结合针刀治疗腰椎间盘突症的优势

Treatment of lumbar disc herniation with percutaneous intervertebral foramen combined with needle knife

目的探讨经皮椎间孔镜结合针刀治疗腰椎间盘突出症的优势。方法前瞻性选取2011年4月至2017年4月住院治疗的腰椎间盘突出症患者60例,随机分为两组。对照组应用经皮椎间孔镜治疗,研究组应用经皮椎间孔镜结合针刀治疗。比较两组患者治疗前、治疗后1周、2周、4周、8周、12周通过日本骨科学会腰痛(JOA)评分、视觉模拟法评分(VAS)以及Oswestry功能障碍指数(ODI)。结果治疗前两组患者的JOA评分、VAS评分和ODI指数无显著差异(P>0.05),治疗后两组患者均得到一定改善,且研究组治疗后各时间点各观察指标均显著优于对照组(P<0.05)。结论经皮椎间孔镜联合针刀治疗腰椎间盘突出症,有利于改善治疗效果,减轻患者疼痛情况。

Objective To evaluate the advantages of percutaneous lumbar interbody combined with needle knife in the treatment of lumbar disc herniation. Methods Sixty patients with lumbar disc herniation treated from April 2011 to April 2017 were randomly divided into two groups. The control group was treated with percutaneous intervertebral foramen. The study group was treated with percutaneous intervertebral foramen combined with needle knife. The JOA score,VAS score and Oswestry Disability Index（ ODI） were compared between the two groups before treatment,1 week,2 weeks,4 weeks,8 weeks and 12 weeks after treatment. Results There was no significant difference in JOA score,VAS score and ODI index between the two groups before treatment（ P 〉0. 05）. After treatment,both groups got a certain improvement,and the study group was significantly better than the control group at each time point after treatment（ P 〈 0. 05）. Conclusion The combination of percutaneous lumbar intervertebral disc and needle knife treatment in patients with lumbar disc herniation is helpful to improve the therapeutic effect and reduce the pain of the patients.
