气道内金属支架置入术联合布地奈德雾化吸入治疗重度良性中央气道狭窄的效果 被引量:1

Effect of Endobronchial Metal Stent Implantation Combined with Budesonide Atomization Inhalation on Severe Benign Central Airway Stenosis

摘要 目的探讨气道内金属支架置入术联合布地奈德雾化吸入治疗重度良性中央气道狭窄的临床效果。方法将50例重度良性中央气道狭窄患者按随机数字表法分为对照组和研究组,每组25例。2组均行气道内金属支架置入术,并在1个月后取出。在此基础上,研究组术后采用布地奈德混悬液雾化吸入治疗。比较2组介入手术前和介入手术1、6个月后气道直径。结果 2组均随访6个月,发生中央气道再狭窄9例,其中研究组3例(均为支气管结核),对照组6例(气管插管1例、气管切开后再狭窄2例、支气管结核3例),均再次行介入手术后症状缓解。研究组介入手术6个月后气道直径>对照组(P<0.05)。结论与单纯气道内金属支架置入术比较,布地奈德雾化吸入联合气道内金属支架置入术治疗重度良性中央气道狭窄具有良好的疗效,能降低再狭窄程度。 Objective To explore the clinical effect of endobronchial metal stent implantation combined with budesonide atomization inhalation on severe benign central airway stenosis.Methods Fifty patients with severe benign central airway stenosis were randomly divided into control group and study group,with 25 patients in each group.Both groups were treated with endobronchial metal stent implantation for 1 month.Additionally,the study group received budesonide aerosol inhalation after operation.Airway diameter was compared between the 2 groups before and after 1 and 6 months of intervention.Results All patients were followed up for 6 months.A total of 9 patients had central airway restenosis,including 3 patients with bronchial tuberculosis in study group and 6 patients in control group(1 case of tracheal intubation,2 cases of restenosis after tracheotomy and 3 cases of bronchial tuberculosis).All the symptoms were relieved after repeated intervention.The diameter of airway in study group was greater than that in control group after 6 months of intervention(P<0.05).Conclusion The combination of budesonide atomization inhalation and endobronchial metal stent implantation is superior to endobronchial metal stent implantation alone for treating severe benign central airway stenosis and reducing the degree of restenosis.
