Expression and Significance of PHLPP1 and PHLPP2 in GIST

目的:分析PHLPP1与PHLPP2在胃肠间质瘤(GIST)中的表达及意义,为其临床治疗与预后评估提供新靶点。方法:以某院收治的30例GIST患者为研究对象,根据术后病理诊断结果分为极低危组9例、低危组13例和中高危组8例,所有患者均于术中留取病理组织和周边健康组织,采用免疫组织化学实验测定PHLPP1与PHLPP2的表达水平,并统计患者临床诊治与随访资料,观察PHLPP1与PHLPP2的表达水平与患者预后的关系。结果:极低危组患的PHLPP1与PHLPP2的表达水平高于低危组,低危组高于中高危组,任意两组的差异均有统计学意义。术后随访1~3年,中高危组的预后不良发生率明显高于极低危组,组间差异有统计学意义(P<0.05),预后不良患者的PHLPP1与PHLPP2表达水平明显低于预后良好者,差异有统计学意义(P<0.05)。ROC曲线分析结果显示,PHLPP1与PHLPP2表达水平对预后不良均有预测价值(P<0.05),其最佳工作点分别为1.810和2.625。结论:PHLPP1与PHLPP2表达水平与GIST的侵袭行为密切相关,对患者预后具有一定的预测价值。

Objective:To analyze the expression and significance of PHLPP1 and PHLPP2 expression in GIST,so as to provide new target for its clinical treatment and prognosis evaluation.Methods:Totally 30 GIST patients were selected as study objects,which were divided into extremely low risk group(9 cases),low-risk group(13 cases),and middle and high risk group(8 cases)according to diagnose results.The pathological tissues and peripheral healthy tissues were taken from all patients during the operation,the immunohistochemical assay was used to detect the expression of PHLPP1 and PHLPP2,and the clinical diagnosis,treatment and follow-up data were analyzed.Further more,the relationship between the expression of PHLPP1 and PHLPP2 and the prognosis of patients was observed.Results:The PHLPP1 and PHLPP2 expression level of extremely low-risk group was higher than that of low-risk group,and the PHLPP1 and PHLPP2 expression level of low-risk group was higher than that of the middle and high risk group,and the differences between any two groups were statistically significant.After 1~3 years of follow-up,the incidence of poor prognosis in the middle and high risk groups was significantly higher than that in the extremely low risk group.There was a significant difference between the two groups(P <0.05).The PHLPP1 and PHLPP2 expression level of patients with poor prognosis were lower than that of patients with good prognosis,the difference between the two groups had statistical significance(P<0.05).The ROC analyse results showed that both PHLPP1 and PHLPP2 expression level had predictive value for poor prognosis,and their best operating point was 1.810 and 2.625 respectively.Conclusion:The PHLPP1 and PHLPP2 expression levels are closely related to invasion behavior of GIST,and have predictive value for the prognosis of patients.
