梗死前心绞痛对急性心肌梗死的影响研究

Effect of Angina Pectoris Before Infarction on Acute Myocardial Infarction

目的观察急性心肌梗死发病前出现心绞痛患者的预后效果,与未出现心绞痛的患者资料进行对比。方法抽取我院2016年1—12月收治的73例急性心肌梗死前心绞痛发病患者,作为研究组,并另选取73例单纯出现急性心肌梗死的患者作为对照组,对比两组预后效果。结果研究组不良事件发生率8.21%,心功能不全3例,心律失常2例,死亡1例,对照组不良事件发生率31.51%,心功能不全8例,心源性休克6例,心律失常7例,死亡2例,两组心肌酶峰值、梗死面积、住院时间差异均具有统计学意义(P<0.05)。结论急性心肌梗死患者出现心绞痛的发病率较高,急性心肌梗死前36 h出现心绞痛,能够起到保护心肌的作用,属于心脏预适应机制,对急性心肌梗死患者的预后有帮助。

Objective To observe the prognosis of patients with angina pectoris before acute myocardial infarction, and to compare with data of patients without angina pectoris. Methods From January 2016 to December 2016, 73 cases of angina pectoris before acute myocardial infarction in our hospital were selected as the experimental group, and the other 73 cases of patients with acute myocardial infarction were selected as control group, the prognosis effects were compared between two groups. Results The incidence of experimental group was 8.21%, 3 cases of cardiac insuffciency, 2 cases of arrhythmia, 1 case of death. The incidence of adverse events was 31.51% in the control group, 8 cases of cardiac insuffciency, 6 cases of cardiogenic shock, 7 cases of arrhythmia, 2 cases of death, peak value of myocardial enzyme, infarct size and hospitalization time had statistical significance difference between groups （P 〈 0.05）. Conclusion The incidence of angina pectoris in patients with acute myocardial infarction is high, the onset of angina pectoris 36 hours before acute myocardial infarction, can play the role of myocardial protection, it is belonging to the cardiac preconditioning mechanism, and is helpful to the prognosis of patients with acute myocardial infarction.
