家庭医生签约服务在老年慢性疾病健康管理中的应用效果

Application of Family Doctor Signing Service in the Health Management of Chronic Diseases in the Elderly

目的分析在老年慢性疾病健康管理中实施家庭医生签约服务的应用效果。方法选取2015年9月—2017年1月本社区医院门诊诊治的100例老年高血压、2型糖尿病患者,将参加家庭医生签约服务的50例患者作为观察组,非家庭医生签约服务的50例患者作为对照组。观察比较两组患者干预后血压、血脂、血糖、慢性病知识知晓率和自控能力等指标。结果两组患者在干预前各指标比较差异无统计学意义(P>0.05);干预后,观察组患者SBP、DBP、TC、TG、HDL、LDL、PFG、2 h PG、Hb A1c各指标水平均优于对照组(P<0.05);两组患者干预后,观察组患者对血压、血糖相关知识的知晓率以及对于遵医嘱服药、监测血压和血糖等自控能力优于对照组患者,两组比较差异有统计学意义(P<0.05)。结论实施家庭医生签约服务对老年慢性疾病患者意义重大,更利于改善患者血压、血脂、血糖等指标,提升对于慢性疾病的健康管理效果。

Objective To analyze the application effect of family doctor signing service in the health management of chronic diseases in the elderly. Methods From September 2015 to January 2017, 100 elderly patients with hypertensive and 2 type diabetes in our community hospital were selected. 50 patients who participated in family doctor contracted services were selected as the observation group, and 50 non-family doctors contracted as the control group. The blood pressure, blood lipid, blood sugar, knowledge rate of chronic disease and self-control ability were compared after the intervention of the two groups. Results There was no statistical signifcance difference of indexesin two groups of patients before intervention （P 〉 0.05）; after the intervention, SBP, DBP, TC, TG, HDL, LDL, PFG, 2 h PG, HbA1c indicatorsin the patients in the observation group were signifcantly better than the control group （P 〈 0.05）. The awareness rate of patients on blood pressure, blood sugar and related knowledge for medication, blood pressure and blood glucose monitoring and self-control in the observation group was signifcantly better than the control group, there was signifcant difference between two groups （P 〈 0.05）. Conclusion The implementationof family doctor signing service is significant for elderly patients with chronic diseases, which is more conducive to improving blood pressure, blood lipids and blood sugar, and improving the health management effect for chronic diseases.
