窗口药师发药时间与发药差错间的关系探索

Study on the Relationship Between Drug Delivery Time and Medication Errors of Window Pharmacists

目的探索窗口药师最佳发药时长,提高窗口药师发药效率,最大限度降低窗口药师发药差错,提高患者用药安全性。方法详细采集2017年第一季度我院门诊药房发药差错明细,通过对我院窗口药师发药时间及发药差错率进行统计分析,结合文献探索窗口药师最佳发药时长与差错率间的关系,探寻最高效低差错的发药时长,为窗口药师安全、高效的工作提供依据。结果窗口药师复核发药时间在1.5h以内效率最高、差错率较低,发药时间在2 h及以上工作效率下降、差错率升高。结论建议窗口药师复核发药时间控制在1.5 h以内是较为合理的时间安排;合理安排窗口药师发药时长、优化工作环节、降低发药差错,药师责任重大。

Objective To explore the best drug delivery time of window pharmacists, to improve the drug delivery efficiency of the window pharmacists, to minimize the drug dispensing errors of the window pharmacists, and to improve the safety of medication. Methods With the acquisition of dispensing error details in outpatient pharmacy in our hospital during the first quarter of 2017, dispensing time of window pharmacists and dispensing error rate was statistically analyzed, the relationship between best dispensing time of window pharmacists and the error rate was explored through literature, the best dispensing time with most efficient and low mistake rate was find out, so as to provide the basis of efficient and safe work for window pharmacist. Results When the time of drug reexamination was less than 1.5 h, the effciency was the highest, and the error rate was lower, when the drug delivery time was over 2 h, the work effciency decreased, and the error rate increased signifcantly. Conclusion It is reasonable for the pharmacist to reexamine the drug within 1.5 h. It is important for pharmacists to arrange the time of drug delivery, optimize the working procedure and reduce the dispensing errors.
