延续性护理干预在老年冠心病合并慢性高血压患者中的应用

Application of continuous nursing intervention for elderly coronary heart disease patients complicated with chronic hypertension

目的研究延续性护理干预措施在老年冠心病合并慢性高血压患者中的应用及对患者心功能、血压水平的影响。方法选取本院接受诊治的98例老年冠心病合并慢性高血压患者作为研究对象,根据随机数表法分为对照组和研究组各49例。对照组接受常规的出院指导,研究组在对照组护理基础上实施延续性护理干预。比较2组患者干预前、干预后3个月心功能、血压水平及心理健康水平变化。结果 2组患者干预后的左心室射血分数(LVEF)水平、6 min步行距离均高于干预前,且研究组高于对照组(P<0.05);干预后2组患者的左室舒张末期内径(LVEDd)水平均低于干预前,且研究组低于对照组(P<0.05);干预后2组患者的舒张压(DBP)、收缩压(SBP)水平均低于干预前,且研究组低于对照组(P<0.05);干预后2组患者的汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表(HAMD)水平均低于干预前,且研究组低于对照组(P<0.05)。结论延续性护理干预措施能有效改善冠心病合并慢性高血压患者的心功能,控制血压水平平稳,提高心理健康水平,值得临床推广。

Objective To study the application of continuous nursing intervention for elderly coronary heart disease patients complicated with chronic hypertension and its effect on cardiac function and blood pressure. Methods A total of 98 elderly patients with coronary heart disease complicated with chronic hypertension treated in our hospital were divided into control group and study group according to random number table method,with 49 cases in each group. The patients in the control group were given regular discharge guidance,and the patients in the study group were given continuous nursing intervention on the basis of the control group. The changes of cardiac function,blood pressure level and mental health level were compared between the two groups before and at 3 months after intervention. Results The levels of LVEF and 6 min distance after intervention in both groups were higher than intervention before,and study group was higher than the control group( P < 0. 05).The levels of LVEDd,DBP,and SBP after intervention in both groups were lower than intervention before,and the study group was lower than the control group( P < 0. 05). The levels of HAMA and HAMD in both groups after intervention were lower than intervention before,and the study group was lower than the control group( P < 0. 05). Conclusion Continued nursing intervention can effectively improve cardiac function in patients with coronary heart disease complicated with chronic hypertension,control the blood pressure level and improve the level of mental health,so it is worthy of clinical promotion.
