胸腔镜切除术后组织EGFR和IGF-1R蛋白表达与胸腺瘤患者化疗预后的关系分析

摘要 目的探讨胸腺瘤患者胸腔镜切除术后组织表皮生长因子受体(EGFR)和类胰岛素样生长因子受体(IGF-1R)表达水平与其化疗预后的关系分析。方法选取2014年7月~2017年7月我院收治50例胸腺瘤患者作为研究对象,均给予胸腔镜切除术联合放射治疗,应用免疫组化En Vision两步法检测50例胸腺瘤组织中EGFR和IGF-1R的表达,随访生存资料并分析患者预后情况。结果存在EGFR和IGF-1R阳性表达的患者分别为25例(52.00%)和28例(56.00%)。EGFR阳性表达病例的3年总生存率显著低于EGFR阴性表达病例(P<0.05);IGF-1R阳性表达病例的3年总生存率显著低于EGFR阴性表达病例(P<0.05);EGFR阳性表达病例的3年无复发生存率显著低于EGFR阴性表达病例(P<0.05);IGF-1R阳性表达病例的3年无复发生存率显著低于EGFR阴性表达病例(P<0.05)。结论胸腺瘤组织中存在EGFR和IGF-1R阳性表达的患者往往预后不佳。 Objective To discussing the relationship between EGFR and IGF-1R in video-assisted thoracoscopic extended thymectomy and Radiotherapy prognosis in patients with thymoma. Methods A total of 50 patients with thymoma were selected from July 2014 to July 2017 in our hospitals,it given with video-assisted thoracoscopic extended thymectomy combined radiotherapy treatment.Immunohistochemistry EnVision two-step method was used to detect the expression of EGFR and IGF-1R in 50 cases of thymoma.The Survival data were followed up and analyzed among different groups. Results EGFR and IGF-1R were found in 25 (52.00%)and 28 (56.00)thymoma,respectively.The overall 3-year survival rates in patients with EGFR positive expression were significantly lower than those in negative expression(P 〈0.05);The overall 3-year recurrent-free survival in patients with IGF-1R positive expression were significantly lower than those in negative expression(P 〈0.05);The overall 3-year survival rates in patients with EGFR positive expression were significantly lower than those in negative expression(P 〈0.05);The overall 3-year recurrent-free survival in patients with IGF-1R positive expression were significantly lower than those in negative expression(P 〈0.05). Conclusion Among tumor tissue,EGFR and IGF-1R is associate with worse survival in patients with thymoma.
