摘要
目的研究大承气汤对急性重症胰腺炎(SAP)大鼠血清白细胞介素-6(IL-6)、可溶性白细胞介素-2受体(s IL-2R)水平的影响,探讨其对胰腺腺泡细胞凋亡的作用及SAP的发病机制。方法将66只成年健康雌性SD大鼠[平均体重(230±20)g]随机分为空白组(6只)、模型组(30只)、大承气汤组(30只)。模型组及大承气汤组采用腹腔注射L-精氨酸方法造模,空白组腹腔注射等量生理盐水。模型组及大承气汤组分别按时间点2、6、12、24、48 h进行分组,每组6只。在造模成功后分别将模型组和大承气汤组SD大鼠按照对应时间点处死并进行分批股动脉取血及手术取胰腺组织。采用酶联免疫吸附试验(ELISA)检测IL-6、s IL-2R水平,使用流式细胞仪检测胰腺腺泡细胞凋亡发生情况。结果大承气汤组及模型组大鼠组内2 h与12、24、48 h时的血清IL-6、s IL-2R水平比较,差异均有统计学意义(P<0.05);大承气汤组大鼠12、24、48 h时的血清IL-6、s IL-2R水平与模型组比较,差异均有统计学意义(P<0.05)。大承气汤组大鼠2、6、12、24、48 h的胰腺腺泡细胞凋亡率均显著高于模型组,差异均有统计学意义(P<0.05)。结论大承气汤可以通过调节血清IL-6、s IL-2R水平诱导胰腺腺泡细胞凋亡,从而减轻SAP的严重程度,缓解重症胰腺炎症状。
Objective To study the effect of Dachengqi decoction on levels of serum IL-6 and soluble interleukin-2 receptor(s IL-2 R)and the apoptosis of pancreatic acinar cells in rats with severe acute pancreatitis(SAP),and explore the mechanism of SAP. Methods A total of 66 adult healthy female SD rats,with average weight of(230±20)g,were randomized into blank group(n=6),model group(n=30)and Dachengqi decoction group(n=30). The rats in the model group and the Dachengqi decoction group were with intraperitoneal injection of L-arginine for modeling,while the blank group was with intraperitoneal injection of equivalent normal saline. Subgroups were conducted in the model group and the Dachengqi decoction group on 2,6,12,24,48 h,6 rats in each subgroup. After the model was established,the blood was collected from the femoral artery and the pancreatic tissue was obtained by laparotomy after execution according to the corresponding time. The levels of IL-6 and s IL-2 R were detected by ELISA method,and the apoptosis of pancreatic acinar cells was detected by flow cytometry. Results Compared with in 2 h,the difference on levels of serum IL-6 and s IL-2 R in the Dachengqi decoction group and the model group in12,24,48 h had statistical significance(P<0.05);while the difference on levels of serum IL-6 and s IL-2 R between the Dachengqi decoction group and the model group in 12,24,48 h had statistical significance(P<0.05). The apoptosis rates of pancreatic acinar cells in Dachengqi decoction group on 2,6,12,24,48 h were obviously higher than those in the model group,and the difference has statistical significance(P<0.05). Conclusion Dachengqi decoction can reduce the severity and relieve the symptoms of SAP by regulating the levels of serum IL-6 and s IL-2 R and inducing apoptosis of pancreatic acinar cells.
