糖尿病胫前色素斑与2型糖尿病患者心血管事件发病风险关系的队列研究

Relationship between pigmented pretibial patches and the risk of cardiovascular events in type 2 diabetes mellitus:aprospective cohort study

目的探讨糖尿病胫前色素斑(PPP)与T2DM并发症进展的关系。方法选取T2DM患者207例,根据是否合并PPP分为糖尿病胫前色素斑组(PPP)和无糖尿病胫前色素斑组(NPPP)。随访6年,随访结束时共脱落64例,比较完成随访的两组(PPP组111例,NPPP组32例)糖尿病并发症进展情况。Spearman相关性分析PPP与各指标的相关性,多元Logistic回归分析新发心血管事件的危险因素。结果 Spearman相关性分析显示,PPP与糖尿病病程呈正相关,与SBP呈负相关(P<0.05)。PPP组新发心脑血管事件发生率(49.55%)高于NPPP组(28.13%)(P=0.032)。多元Logistic回归分析显示,PPP(β=0.009,P=0.027)、HbA1c(β=0.462,P=0.019)、SBP(β=0.047,P=0.033)为新发心血管事件的危险因素。PPP与T2DM患者新发心血管事件关系密切[OR1.009,95%CI(1.001,1.017)]。结论 PPP与T2DM患者新发心血管事件有相关性。

Objective To evaluate the association between pigmented pretibial patches（PPP）and the progression of complications in type 2 diabetes（T2DM）.Methods A total of 207 patients with T2DM were enrolled in this study and divided into 2 groups：patients with PPP（PPP group）and patients without PPP（NPPP group）.All the patients were followed up for 6 years,and 64 patients lost follow up at the end of the study.The clinical characteristics and biochemical parameters were compared between PPP（n=111）and NPPP groups（n=32）.Spearman correlation analysis was used to evaluate the relationship between PPP and other factors.Multiple logistic regression analysis was used to examine the risk factors for new cardiovascular events.Results Spearman correlation analysis revealed that PPP was positively correlated with diabetes duration,negatively correlated with SBP（P〈0.05.The incidence of cardiovascular events was significant higher in PPP group（49.55%）than in NPPP group（28.13%）（P=0.032）.Multiple Logistic regression analysis further revealed that PPP（β=0.009,P=0.027）,HbA1c（β=0.462,P=0.019）,and SBP（β=0.047,P=0.033）were independent risk factors for cardiovascular events in T2DM.PPP was related to new cardiovascular events in patients with T2DM[OR1.009,95%CI（1.001,1.017）].Conclusion PPP is associated with new cardiovascular events in patients with T2DM.
