微创切口结合带线锚钉改良Krackow缝合法治疗闭合性跟腱断裂

Minimally invasive incision combined with wire anchor pin modified Krackow stitch method in the treatment of closed achilles tendon rupture

目的探讨微创切口结合带线锚钉改良Krackow缝合法治疗闭合性跟腱断裂的临床应用价值。方法选取2010年7月~2017年3月收治的80例闭合性跟腱断裂患者作为本次研究对象,将其分为A、B两组,各40例。A组采用常规跟腱内侧切口结合Krackow缝合法进行跟腱修补,B组采用微创切口结合锚钉改良Krackow缝合法治疗闭合性跟腱断裂,比较两组患者的出血量、切口长度、手术时间、住院天数、愈合时间及临床疗效。临床疗效均采用Arner-Lindholm评定法进行疗效评定。结果 B组患者术中出血量(47.77±10.34)mL,切口长度为(4.21±1.18)cm,手术时间为(40.56±11.45)min,住院时间为(8.69±3.18)d,愈合时间(5.28±1.16)周,且总优良率为92.50%,均优于A组(P<0.05)。结论应用微创切口结合带线锚钉改良Krackow缝合法修复闭合性跟腱断裂手术切口小,手术时间短,固定可靠,可早期功能锻炼,术后并发症少,值得临床推广应用。

Objective To investigate the clinical application value of minimally invasive incision combined with wire anchor pin modified Krackow stitch method in the treatment of closed rupture of achilles tendon rupture. Methods 80 patients with closed achilles tendon rupture who were treated from July 2010 to March 2017 were selected as the study objects and divided into two groups including group A（n=40） and group B（n-40）. The group A underwent achilles tendon repair by conventional medial incision of achilles tendon combined with Kraekow suture method. The group B was treated by minimally invasive incision combined with anchor nail modified Kracknw suture method for closed rupture of achilles terldon rupture. The bleeding volume, incision length, operative time, hospitalization time, healing time and clinical curative effect between the two groups were compared. The clinical curative effect was evaluated by AruerLindholm assessment. Results The amount of bleeding during the operation was （47.77±10.34） mL, the incision length was（4.21±1.18） cm, the operation time was（40.56±11.45） rain, the length of hospitalization was （8.69±3.18） d, the healing time was （5.2±1.16） weeks, and the total exeellent and good rate was 92.50% in group B, all better than those in the group A（P〈0.05）. Conclusion The application of minimally invasive incision combined with wire anchor pin modified Krackow stitch method in repairing closed achilles tendon rupture has small incision, with short operation time, reliable, early functional exercise and less postoperative complications, which is worthy of clinical application.
