摘要 目的分析CT引导^(125)I粒子植入治疗非小细胞肺癌近期疗效的影响因素。方法选取符合本次研究相关标准的非小细胞肺癌患者150例;均给予放射性^(125)I粒子植入治疗,评估其疗效,统计RR率,将年龄、性别、发病部位、肿瘤直径、病理类型等因素作为可能影响治疗效果的因素纳入研究,先后进行单因素和先Cox多因素回归分析。结果不同发病部位、肿瘤直径、化疗情况下RR率差异显著(P<0.05);将近期疗效作为应变量,将单因素分析中有意义的因素代入Cox回归模型分析,发现TNM分期(P=0.000)、肿瘤直径(P=0.001)、D100(P=0.003)、化疗情况(P=0.002)是影响疗效的主要因素,TNM、肿瘤直径与RR率为负相关,D100<80 Gy则RR降低,当D100≥80 Gy时RR率提高。结论 CT引导^(125)I粒子植入能有效治疗非小细胞肺癌近期疗效影响因素较多,可根据患者是否有以上因素预估疗效,指导治疗。 Objective To analyze the influential factors of short-term effect of CT guided 125^I particles implantation in non-small cell lung cancer. Methods A total of 150 patients with non-small cell lung cancer who met the relevant standards of this study were enrolled. All patients were given radioactive 125^I particles implantation. The efficacy was evaluated and the RR rate was calculated. The factors including age, gender, disease location, tumor diameter and pathological type were included in the study as a possible influencing factors of the treatment effect, followed by univariate and prior Cox multivariate regression analysis. Results There were significant differences in RR rates at different sites, tumor diameter and chemotherapy(P〈0.05). The short-term curative effect was taken as the dependent variable, and the significant factors of univariate analysis were substituted into Cox regression model. It was found that TNM stage(P= 0.000), tumor diameter(P=0.001), D100(P=0.003) and chemotherapy(P=0.002) were the main factors influencing the curative effect. TNM, tumor diameter and RR were negatively correlated. RR decreased when D100〈80 Gy,RR increased when D100 980 Gy. Conclusion There are many influencing factors of the short-term curative effect of CTguided 125^I panicles implantation in the effective treatment of non-small cell lung cancer. The efficacy can be predicted according to whether patients have the above factors to guide treatment.
