EB病毒DNA检测对儿童传染性单核细胞增多症的诊断价值及其影响因素 被引量:12

摘要 目的探讨EB病毒(EBV)DNA在儿童传染性单核细胞增多症(传单)中的诊断价值及其影响因素。方法选择2014年2月至2015年9月本科收治的60例儿童传单,比较EBV—DNA、EBV衣壳抗原(CA)IgA、异型淋巴细胞阳性率及阳性病例的年龄、性别和热程。结果EBV—DNA阳性率为81.67%,显著高于其他两种检测方法,检测阳性所需热程最短。热程1~5dEBV—DNA阳性率显著高于其他两种方法,热程11~15d三种检测方法阳性率比较差异无统计学意义。随热程延长,EBV—DNA阳性率及拷贝数逐渐降低,阳性率与热程呈显著负相关。结论EBV—DNA可用于儿童传单早期诊断,优于EBV—CA—IgA和异型淋巴细胞,尤其适合热程小于5d患儿。 Objective To evaluate the diagnostic value of Epstein-Barr virus(EBV) DNA in children with infectious mononucleosis(IM) and its influencing factors. Methods Children with IM hospitalizd into our department from February, 2014 to September, 2015 were enrolled. The positive rates of EBV-DNA, EBV capsid antigen (CA)-IgA, and atypical lymphocyte were compared. The age, gender, and febrile period were compared within positive cases. Results The positive rate of EBV-DNA was 81.67%, which was higher than those of other methods. The febrile period of positive cases for EBV-DNA was the shortest. The positive rate of EBV-DNA for febrile period 1 to 5 days was higher than those of other methods, but there were no statistical difference between the methods for febrile period 11 to 15 day. The loads and positive rate decreased with febrile period; moreover, the positive rate of EBV-DNA negatively related with febrile period. Conclusion EBV-DNA is superior to EBV- CA-IgA and atypical lymphocyte in the early diagnosis of EBV infection in childdren with IM, especially in febrile period less than 5 days.
