医护与康复师共同查房在脑卒中偏瘫患者康复中的应用效果

Effect of doctors and nurses cooperated with rehabilitative therapists making the rounds of the wards on rehabilitation in stroke patients with hemiplegia

目的探讨医护与康复治疗师共同查房模式在脑卒中偏瘫患者康复训练中的应用效果。方法选择在本院神经内科住院符合脑卒中诊断标准的偏瘫患者150例为研究对象，按患者住院时间顺序进行分组，将2016年1—6月实施传统护理查房模式75例患者设为对照组，2016年7—12月实施医护与康复治疗师共同查房模式75例患者设为实验组。采用Fu甜一Meyer肢体运动功能评定量表和Barthel指数评定量表对两组患者出院时肢体功能及日常生活自理能力进行评价、对比两组平均住院时间与住院费用。结果实验组Fusl—meyer肢体运动功能评定量表评分及Barthel指数量表评分均明显高于对照组[（69．78±8．25）分比（51．93±6．98）分和（83．26±6．97）分比（71．35±6．60）分]，平均住院时间及住院费用均明显少于对照组[（22．11±6．39）d比（31．37±9．14）d和（9576．81±524．33）元比（12810．56±751．65）元]，均P〈0．01。结论医护与康复治疗师共同查房模式能明显促进脑卒中患者偏瘫肢体的康复，提高患者的生活自理能力，改善患者生活质量。

Objective To explore the effect of doctors and nurses cooperated with rehabilitative therapists making the rounds of the wards on the rehabilitation in stroke patients with hemiplegia. Methods 150 patients with hemiplegia after stroke treated at the department of neurology of our hospital were selected as the research objects. According to the hospitalization time, 75 patients undertaking traditional mode of rounds from January to June, 2016 were set as a control group and 75 undertaking the rounds made by doctors and nurses cooperated with rehabilitative therapists from July to December, 2016 an experimental group. The two groups＇ physical function and daily living ability were estimated using Fugl-Meyer limb motor fimction rating scale and Barthel index rating scale when discharging. The average hospital stay and the hospitalization expense were compared between the two groups. Results The scores of Fugl-meyer body motor function rating scale and Barthel index rating scale were significantly higher, the hospital stay was shorter, and the hospitalization expense was significantly lower in the experimental group than in the control group [（69.78±8.25） vs. （51.93±6.98）, （83.26±6.97） vs. （71.35±6.60）, （22.11±6.39） d vs. （31.37±9.14） d, and （9 576.81±524.33） yuan vs. （12 810.56±751.65） yuan; all P 〈 0.05]. Conclusions Doctors and nurses cooperated with rehabilitative therapists making the rounds of the wards can significantly promote brain stroke patients with hemiplegia＇s limb rehabilitation and improve their self-care ability and quality of life.
