丙戊酸钠联合止痫汤治疗儿童癫痫部分性发作的合理应用研究

Sodium valproate combining Zhixian decoction in treatment of children with partial seizures of epilepsy

目的研究探讨丙戊酸钠联合止痫汤治疗儿童癫痫部分性发作的合理应用。方法选取2015年1月至2016年6月本院收入儿童癫痫部分性发作患者85例，按癫痫发作的国际分类诊断标准，所有患者均符合儿童癫痫诊断标准。随机分为丙戊酸钠组和中西药联合组。丙戊酸钠组：给予患者口服丙戊酸钠口服液15～20mg/（kg·d），2次，d，症状控制则维持此剂量，症状未控制则增加剂量至20～30mg/（kg·d），2次／d，1年为1个疗程；中西药联合组：在丙戊酸钠组基础上，增加口服止痫汤，1年为1个疗程。结果两组患者治疗后临床治疗效果比较，中西药联合组总有效率（95．35％）明显高于丙戊酸钠组的总有效率（78．57％），差异有统计学意义（P〈0．05）；两组患者治疗后，1年内平均发作次数和平均发作时间均明显减少，差异均有统计学意义（均P〈0．05）；治疗后，中西药联合组平均发作次数（12．85±4．18）次、平均发作持续时间（2．83±0．61）min／次，明显低于丙戊酸钠组的（16．48±9．53）次、（4．12±1．44）min／次，差异均有统计学意义（均P〈0．05）；两组不良反应发生率比较，差异无统计学意义（P〉0．05）。结论采用丙戊酸钠联合止痫汤治疗儿童癫痫部分性发作具有很好的临床疗效，安全性高、操作简便，在临床上具有较好的应用价值。

Objective To study the effect of sodium valproate combining Zhixian decoction in the treatment of children with partial seizures of epilepsy. Methods 85 children with partial seizures of epilepsy treated at our hospital from January, 2015 to June, 2016. According to the international classification of epilepsy diagnosis standard, all the patients were in line with the criteria for the diagnosis of children epilepsy. The children were randomly divided into a sodium valproate group and a Chinese and western medicine combined group. The sodium valproate group took sodium valproate sustained-release tablet, 15-20 mg/（kgod）, 2 times a day and maintained the dose if the symptoms were controlled and the dose was inereased to 20-30 mg/（kg·d） 2 times a day if not controlled, for a year. In addition, the combination group took oral Zhixian decoction for a year. Results The total effective rate was 95.35% in the combination group and 78.57% in the sodium valproate group, with statistical difference （P 〈 0.05）. The average attacks were fewer and the average onset time was shorter after than before the treatment in both groups, with statistical differences （P 〈 0.05）. After the treatment, the seizure attacked （12.85±4.18）times and the seizure duration was （2.83±0.61） min in the combination group and （16.48±9.53）times and （4.12±1.44）min in the sodium valproate group, with statistical differences （P〈0.05）. After the treatment, there was no statistical difference in the incidence of adverse reactions between the two groups （P〉0.05）. Conclusions Sodium valproate combining Zhixian decoction in the treatment of children with partial seizures of epilepsy is effective and safe and has good clinical value.
