湿润生肌油纱促肛瘘术创面愈合的疗效及对肉芽组织因长因子mRNA表达的影响

Effect of Shirun Shengji Oil Yarn on Wound Healing of Anal Fistula and Its Effect on Expression of VEGFmRNA in Granulation Tissue

目的：观察湿润生肌油纱对肛瘘术创面愈合的效果以及对肉芽组织血管内皮生长因子（VEGF）mRNA表达的影响。方法：单纯性肛瘘患者随机分为观察组、对照组和凡士林组各30例,术后分别采用湿润生肌油纱、龙珠软膏纱条和凡士林覆盖手术创口,比较3组创面愈合率、创面愈合时间和创面肉芽生长情况,RT-PCR法测定3组患者肉芽质VEGFmRNA的表达。结果：观察组术后7、14、21 d在创面愈合率和创面肉芽生长积分显著优于凡士林组（P〈0.05）,术后14、21 d创面愈合率和肉芽生长积分显著优于对照组（P〈0.05）;3组创面平均愈合时间分别为（15.23±5.29）d、（19.80±5.57）d和（22.19±4.81）d,观察组创面愈合时间显著少于对照组和凡士林组;术后7 d3组患者肉芽组织VEGFmRNA表达量与GADPH平均比值分别为（0.886±0.088）、（0.781±0.098）、（0.553±0.074）,观察组VEGFmRNA表达量显著高于对照组和凡士林组。结论：湿润生肌油纱可促进创面愈合,改善肉芽组织生长,其机制与上调VEGFmRNA表达有关。

Objective： To observe the effect of Shirnn Shengji Oil Yarn on the wound healing of anal fistula and the effect of vascular endothelial growth factor （VEGF） mRNA expression on granulation tissue. Methods： Patients with simple anal fistula were randomly divided into observation group, control group and Vaseline group, 30 cases in each. The wound healing rate, wound healing time and the growth of granulation were compared within all groups. The expression of VEGF mRNA in granulosa was measured by RT - PCR 7 d after operation. Results ： The wound healing rate and wound granulation growth score of observation group were significantly better than those of the Vaseline group at 7, 14 and 21 days after operation （ P 〈 0. 05 ）, which was significantly better than those of control group at 7d and 21d （ P 〈 0. 05 ）. The mean heafing time of the wound was （ 15.23 ± 5.29） d, （ 19.80 ± 5.57 ） d and （22.19 ± 4.81 ） d, respectively and the observation group＇ s was significant （ P 〈0. 05 ）. The expressions of VEGF mRNA were （0. 886 ± 0.088 ）, （0.781 ± 0.098） and （0. 553 ± 0. 074） respectively. The expression of VEGF mRNA in observation group was significantly higher than that in control group and Vaseline group. Conclusions： Shirun Shengji Oil Yarn promoted wound healing and improved granulation tissue growth after anal fistula by up -regulating VEGFmRNA expression.
