复方丹参滴丸联合炙甘草汤加减治疗老年慢性心力衰竭的临床疗效

Clinical Effect of Compound Danshen Dripping Pills Combined with Modified Zhigancao Decoction in Treating Elderly Patients with Chronic Heart Failure

目的观察复方丹参滴丸联合炙甘草汤加减治疗老年慢性心力衰竭的临床疗效。方法选取2016年6月—2017年6月西安市中医医院收治的老年慢性心力衰竭患者60例,采用随机数字表法分为对照组和研究组,每组30例。在常规治疗基础上,对照组患者给予复方丹参滴丸治疗,研究组患者给予复方丹参滴丸联合炙甘草汤加减治疗;两组患者均连续治疗3个月。比较两组患者临床疗效,治疗前后血清人心型脂肪酸结合蛋白(H-FABP)、基质金属蛋白酶抑制剂1(TIMP-1)水平及心功能指标,并观察两组患者治疗期间不良反应发生情况。结果研究组患者临床疗效优于对照组(P<0.05)。治疗前两组患者血清H-FABP、TIMP-1水平比较,差异无统计学意义(P>0.05);治疗后研究组患者血清H-FABP水平低于对照组,血清TIMP-1水平高于对照组(P<0.05)。治疗前两组患者左心室舒张末期内径(LVEDD)、左心室射血分数(LVEF)、舒张期室间隔厚度(IVST)比较,差异无统计学意义(P>0.05);治疗后研究组患者LVEDD和IVST小于对照组,LVEF高于对照组(P<0.05)。两组患者治疗期间不良反应发生率比较,差异无统计学意义(P>0.05)。结论复方丹参滴丸联合炙甘草汤加减治疗老年慢性心力衰竭的临床疗效确切,可有效改善患者心功能,且安全性较高。

Objective To observe the clinical effect of compound danshen dripping pills combined with modified Zhigancao Decoction in treating elderly patients with chronic heart failure. Methods A total of 60 elderly patients with chronic heart failure were selected in Xi＇an Traditional Chinese Medicine Hospital from June 2016 to June 2017,and they were divided into control group and study group according to random number table, each of 30 cases. Based on conventional treatment,patients in control group received compound danshen dripping pills,while patients in study group received compound danshen dripping pills combined with modified Zhigancao Decoction; both groups continuously treated for 3 months. Clinical effect,serum levels of H-FABP and TIMP-1,and index of cardiac function before and after treatment were compared between the two groups,and incidence of adverse reactions was observed during the treatment. Results Clinical effect in study group was statistically significantly better than that in control group（ P〈0.05）. No statistically significant differences of serum level of H-FABP or TIMP-1 was found between the two groups before treatment（ P〉0.05）; after treatment,serum H-FABP level in study group was statistically significantly lower than that in control group, while serum TIMP-1 level in study group was statistically significantly higher than that in control group（ P〈0.05）. No statistically significant differences of LVEDD,LVEF or IVST was found between the two groups before treatment（ P〉0.05）; after treatment,LVEDD and IVST in study group were statistically significantly smaller than those in control group,while LVEF in study group was statistically significantly higher than that in control group（ P〈0.05）. No statistically significant differences of incidence of adverse reactions was found during the treatment（ P〉0.05）. Conclusion Compound danshen dripping pills combined with modified Zhigancao Decoction has certain clinical effect in treating elderly patients with chronic heart failure,can effectively improve the cardiac function and is relatively safe.
