胃肠道间质瘤临床病理诊断及鉴别诊断

Clinical pathological diagnosis and differential diagnosis of gastrointestinal stromal tumors

目的:分析胃肠道间质瘤临床病理及鉴别诊断。方法:回顾性分析2015年3月~2017年2月期间的11例胃肠道间质瘤患者的临床病理及鉴别诊断情况。结果:11例患者中,7例梭形细胞(63.7%),1例上皮样细胞(9.1%),2种细胞混合型3例(27.2%)。CD_(117)、CD_(34)、平滑肌肌动蛋白(Smooth muscle actin,SMA)、S-100蛋白阳性率分别为90.9%(10/11)、72.8%(8/11)、18.1%(2/11)、9.1%(1/11),desmin均为阴性。结论:胃肠道间质瘤,根据发病部位,CD_(117)与CD_(34)是确诊的重要依据,联合SMA、S-100蛋白以及desmin,可辅助鉴别诊断。

Objective：To analyze the clinical pathology of gastrointestinal stromal tumor and its differential diagnosis.Methods：Retrospective analysis of clinical pathological characteristics and differential diagnosis of 11 cases of gastrointestinal stromal tumors in March 2015~ February 2017.Results：Of the 11 patients,7 had spindle shaped cells（63.7%）,1 epithelioid cells（9.1%）,2 mixed cells,and 3（27.2%）.The positive rates of CD117,CD34,SMA and S-100 protein were 90.9%（10/11）,72.8%（8/11）,18.1%（2/11）and 9.1%（1/11）,respectively,and desmin was negative.Conclusion：Gastrointestinal interstitial tumors,according to the location of the disease,CD_（117） and CD_（34）,are the important basis for the diagnosis.Combined with SMA,S-100 protein and desmin,it can assist in differential diagnosis of gastrointestinal interstitial tumors.
