电子耳蜗植入术患儿围手术期的非语言交流技巧

Nonverbal communication skills in nursing care of pediatric cochlear implant patients

目的探讨电子耳蜗植入术患儿围手术期胡非语言交流技巧。方法选取2012年1月—2016年7月行人工电子耳蜗植入术的患儿84例设为对照组,给予常规护理,选取2016年9月—2017年5月行人工电子耳蜗植入术治疗的患儿56例设为观察组,由经培训的护士完成护理工作,护士系统性地学习术前聋哑患儿的护理知识、非语言交流策略、掌握怀抱等基本亲密接触策略,提高非语言交流质量。观察2组不良事件发生情况。调查观察组患儿依从性和医师对护理的满意度,并与2016年3月—7月20例患儿预调查结果进行比较。结果观察组计划外使用镇定镇静药物、治疗护理路径变异、手术时间推迟的发生率分别为0、3.57%、1.78%,低于对照组的13.10%、16.67%、15.48%(P<0.05)。观察组患儿术前准备、麻醉前穿刺操作、术后苏醒期、术后检查依从性评分均高于基线调查结果(P<0.05),医师对护理满意度高于基线调查结果(P<0.05)。结论高质量的非语言交流有助于提高电子耳蜗植入术患儿的依从性,保障手术、检查等医疗和护理工作的顺利开展。

Objective To evaluate the effect of nonverbal communication skills in nursing care of pediatric cochlear implant patient. Methods Totally 84 pediatric cochlear implant patients were selected as the control group between January 2012 and July 2017,with routine care during treatment. Another 56 pediatric cochlear implant patients were enrolled as the observation group between September 2016 and May 2017. Nursing interventions by using nonverbal communication skills were carried out in the observation group. The compliance of pediatric patients to operation and surgeons' satisfactory degree were compared the observation group and 20 patients who received the pilot survey between March 2016 and July 2016. Results The rate of unplanned sedative drug use,changes in clinical path,delayed operation was 0,3. 57% and 1. 78%in the observation group,and was 13. 10% 、16. 67% 、15. 48% in the control group,with significant difference( P < 0. 05). Patients in the observation group had a higher score of compliance with operation preparation,anesthesia,recovery after anesthesia and postoperative check compared with those 20 patients in pilot study( P < 0. 05). The surgeons were more satisfied with the nursing assistance in the control group than those in the pilot study( P < 0. 05). Conclusion Nonverbal communication quality is helpful to improve the compliance of children with cochlear implant surgery.
