摘要
目的探讨链式护理模式对脑卒中吞咽障碍患者生活质量的影响。方法脑卒中伴吞咽障碍患者200例分为观察组和对照组,各100例。对照组给予神经内科常规护理和随访,观察组在对照组基础上遵循优质整体护理理念,实施以专科医生-吞咽专科护士-社区护士-家属-患者为小组的"全人全程"追踪管理,对比观察出院后6个月两组患者的生活质量及遵医行为情况。结果观察组生活质量综合评定问卷(GQOLI-74)中躯体功能、心理功能和社会功能维度评分高于对照组(P<0.01)。观察组患者合理饮食、正确服药、健康锻炼等遵医行为优于对照组(P<0.01)。结论链式护理模式能提高脑卒中吞咽障碍患者出院后的遵医行为及生活质量。
Objective To investigate the influence of chain model of holistic nursing on quality of life in stroke patients with dysphagia. Methods Totally 200 stroke patients with dysphagia were assigned to the observation group( n = 100) and the control group( n = 100). Patients in both two control group received routine care and follow-up. The chain model of holistic nursing consisting of specialist physician,specialist nurses on dysphagia,community health nurses,patients and their family members were carried out. The quality of life and medical compliance behavior were evaluated at 6 months after discharge. Results Patients in the observation group achieved a higher score of physical function,emotional function and social function in GQOLI-74 assessment compared with those in the control group( P < 0. 01). There were more patients in the observation group following the healthy behavior of dietary,medication and exercise compared with those in the control group( P < 0. 01). Conclusion The chain model of holistic nursing is potentially effective to improve the post-discharge medical compliance and quality of life in stroke patients with dysphagia.
