面肌痉挛的静息态脑功能和结构的初步探究 被引量:5

摘要 目的运用磁共振成像技术探讨侧面面肌痉挛(hemifacial spasm,HFS)患者是否存在脑功能和结构上的改变。材料与方法结合静息态功能磁共振成像(resting-state functional magnetic resonance imaging,rs-f MRI)和基于体素的形态学分析方法(voxel-based morphometry,VBM)观测33例偏侧面肌痉挛患者与33例性别匹配的健康对照之间的自发性脑活动和脑结构上的差异。磁共振数据采用Matlab和SPM12软件进行分析,统计分析采用双样本t检验,统计阈值为体素水平P<0.005,团块水平经过FWE检验矫正,P<0.05,大于30个体素的团块将被报道。结果静息态脑功能结果显示,HFS患者在小脑Ⅵ小叶区的局部一致性(regional homogeneity,Re Ho)值显著低于健康对照;VBM结果显示,HFS患者在小脑Ⅵ小叶区灰质体积显著低于健康对照。结论 HFS患者的小脑Ⅵ区存在脑功能和脑结构的改变,提示小脑异常可能和HFS患者持续面部肌肉不自主抽搐的状态有关。 Objective: Using magnetic resonance imaging technique to explore the change of brain function and structure in patients with hemifacial spasm(HFS). Materials and Methods: This paper combined resting-state functional magnetic resonance imaging(rs-f MRI) and voxel-based morphometry(VBM) to investigate the spontaneous brain activity and structural differences between the 33 patients with HFS and the 33 healthy controls of gender matching. And the MRI data were analyzed by SPSS, SPM12 and Matlab. Two-sample t-tests were used to determine betweengroup differences and thresholds at P0.005(voxel level), and FWE were corrected to P〈0.05 at the cluster level. The surviving clusters 30 were reported. Results: Compared to the controls, the patients with HFS exhibited significantly decreased regional homogeneity(Re Ho) value in the cerebellum lobule Ⅵ, and decreased gray matter volume in the cerebellum lobule Ⅵ too. Conclusions: Our results showed that patients with HFS exhibited function and structure alteration in the cerebellum lobule Ⅵ, which would play an important role in pathology of HFS.
