良性前列腺增生行直光束绿激光前列腺汽化术治疗的效果体会

Experience in the Treatment of Benign Prostatic Hyperplasia With Direct Light Green Laser Prostatic Vaporization

目的探讨良性前列腺增生行直光束绿激光前列腺汽化术治疗的效果体会。方法将2016年2月—2017年2月在我院泌尿外科治疗的200例良性前列腺增生患者随机分为两组,对照组采用经尿道前列腺电切术(TURP)治疗,观察组采用直光束绿激光前列腺汽化术(PVP)治疗,比较两组患者的各项手术指标、术后症状改善情况、并发症发生率。结果观察组手术时间、术中出血量、术后膀胱冲洗时间、术后留置导尿管时间、住院时间与对照组相比,差异有统计学意义(P<0.05);观察组术后残余尿量、最大尿流率、IPSS评分与对照组相比,差异无统计学意义(P>0.05);两组在术后血尿、轻度尿痛、尿频尿急、尿道狭窄、排尿困难、性功能障碍等并发症发生率对比,差异无统计学意义(P>0.05)。结论良性前列腺增生行直光束绿激光前列腺汽化术治疗的效果良好,创伤小,术后恢复快,且症状改善确切,并发症发生率低。

Objective To investigate the effect of direct green laser prostatic vaporization for benign prostatic hyperplasia （BPH）. Methods From February 2016 to February 2017, 200 patients with benign prostatic hyperplasia were randomly divided into two groups, which were treated in department of urology in our hospital. The control group was treated with transurethral resection of the prostate （TURP）, and the observation group was treated with direct beam green laser prostatic vaporization （PVP）. The operation indexes of the two groups, the improvement of postoperative symptoms and the incidence of complications were compared. Results The operative time, intraoperative blood loss, postoperative bladder irrigation time, postoperative indwelling catheter time and hospitalization time in the observation group were signifcantly different from those in the control group （P 〈 0.05）. There was no significant difference in the residual urine volume, maximum urine fow rate and IPSS score in the observation group compared with the control group （P 〉 0.05）. The two groups in postoperative mild hematuria, dysuria, frequent urination, dysuria, urethral stricture, sexual dysfunction and other complications of contrast, there was no statistically signifcant difference （P 〉 0.05）. Conclusion Benign prostate hyperplasia treated with direct beam green laser vaporization of prostate is effective, with less trauma, faster postoperative recovery, and exact improvement of symptoms and low incidence of complications.
