康复日记结合护理随访在提高头颈癌放疗患者康复锻炼依从性中的作用研究

Effects of rehabilitation diary and nursing follow-up on rehab training compliance among patients finishing radiotherapy for head and neck cancer

目的探讨康复日记结合护理随访干预对头颈癌放疗患者康复锻炼依从性的影响。方法将84例头颈癌放疗的患者分为干预组和对照组,各42例。对照组患者予常规出院护理及门诊随访,干预组患者在常规出院指导的基础上增加患者康复日记结合护理随访干预。干预方案由干预小组护士于患者出院前3d给予日记记录指导,并在出院后1周、2周、1个月、2个月、3个月时实施电话随访。采用自制"头颈癌患者康复训练依从性问卷"测量两组患者干预前、出院后1个月及3个月的依从性。结果两组患者干预前康复锻炼依从性得分差异无统计学意义(P>0.05);在出院后1个月和3个月康复锻炼依从性得分差异有统计学意义(P<0.05),干预组患者的康复锻炼依从性得分均显著高于对照组(P<0.05)。结论患者康复日记结合护理随访干预有助于提高头颈癌放疗患者康复锻炼的依从性。

Objective To explore the effects of the intervention of rehabilitation diary and nursing follow-up on rehab training compliance among patients receiving radiotherapy for head and neck cancer.Methods 84 patients with head and neck cancer were divided into intervention group(n=42)and control group(n=42).The patients in the control group received a standard discharge nursing,while those in the intervention group received a standard care plus the intervention nursing.The intervention was consisted of health education on using diaries three days before discharge,telephone follow-up in one week,two week,one month,two month and three month after discharge by the nurse case managers.Compliance data was measured at the end of 1month and 3month after discharge by a selfdesigned questionnaire on rehab training compliance for head and neck cancer patients.Results There were no significant difference between the two groups before intervention(P>0.05)The rehab training compliance of the intervention patients were significantly higher than those of the control at both time points.Compared with the end of 1month after discharge,the rehab training compliance at the end of 3month after discharge in both groups were decreased(P<0.05),but scores about the compliance of intervention group are higher than the control group.Conclusion The intervention of patient diary and nursing follow-up can improve rehab training compliance for head and neck cancer patients finishing radiation treatment.
