化痰祛瘀汤对冠心病稳定型心绞痛患者血脂及血液流变学指标的影响

Clinical Observation on the Effect of Huatan Quyu Decoction on Blood Lipid and Hemorheology in Patients with SCAD

目的观察化痰祛瘀汤对冠心病稳定型心绞痛患者血脂及血液流变学指标的影响。方法将80例稳定型心绞痛患者按随机数字表法分为治疗组与对照组各40例。对照组予西医常规治疗,治疗组在常规治疗基础上予化痰祛瘀汤,4周为1个疗程。连续治疗2个疗程后,对两组患者的临床疗效、中医证候疗效、心绞痛CCS分级、血脂及血液流变学指标变化进行统计分析,并观察不良反应。结果治疗组临床疗效、中医证候疗效及心绞痛CCS分级的改善均高于对照组(P<0.05);在血脂水平方面,两组患者治疗后总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)及高密度脂蛋白胆固醇(HDL-C)水平均较治疗前显著下降(P<0.01);组间比较仅TC、HDL-C、LDL-C水平差异具有统计学意义(P<0.05);在血液流变学方面,两组患者治疗后全血高、低切黏度,血浆黏度、纤维蛋白原水平均较治疗前均显著下降(P<0.01),组间差异均有统计学意义(P<0.05);两组间不良反应事件发生情况差异无统计学意义(P>0.05)。结论化瘀祛痰汤对冠心病稳定型心绞痛临床治疗效果显著,有效缓解患者临床症状,降低心绞痛CCS分级,且具有较高的安全性,还可调节血脂水平、改善血液流变学,对于该类患者的预后意义重大。

Objective： To investigate the effect of Huatan Quyu Decoction on blood lipid and hemorheology in patients with SCAD. Methods： 80 patients with SCAD were randomly divided into two groups,and the eontrnl group （n = 40） was treated with conventional Western medicine,while the treatment group （n=40） with Huatan Quyu Decoction fur 4 weeks as a course. After 2 curses of treatment,the clinical efficacy,TCM syndrome scores, CCS grading of angina pectoris,blond lipid and hemorhenh）gy of the patients in the two groups were analyzed,and the adverse reactions were nbserved. Results： Clinical curative effect,TCM syndrome curative effect and the im- provetnent of CCS grading of angina pectoris of the treatment group were higher than those of control group （P〈 0.05）. In terms of blood lipid levels.after treatment,levels of TC,TG,HDL-C and LDL-C were decreased signifi- cantly,compared with before treatment （P〈 0.01 ）. There was significant difference between the groups in the lev- els of TC, HDL-C and LIDL-C （P 〈 0.05）. In terms of hemorheology ,after treatment,whole blood high and low shear rate, plasma viscosily and fibrinogen were lower than those before treatment （P〈 0.01 ）,and the differences between the groups were statistically significant （P〈0.05）. There was no significant difference in the ineidence of adverse events between the Iwo groups （P〉 0.05 ）. Conclusions： Huatan Quyu Decoction on SCAD has a signifi- cant curative effect,and can effectively relieve clinical symptoms,improve the clinical efficacy and reduce the angina CCS grade with great seenrily. Meanwhile,it can also regulate blood lipid levels and improve hemorheolo- gy,which means a lot to patients and is worthy of clinical promotion.
