复方糖肾煎对2型糖尿病肾病大鼠足细胞的保护作用 被引量:1

The Protective Effect of Compound Tangshenjian Decoction on Podocyte of Type 2 Diabetic Nephropathy Rats

摘要 目的观察中药复方糖肾煎对2型糖尿病肾病气阴两虚证大鼠足细胞的保护作用。方法采用随机数字表法将大鼠分为假手术组、模型组、糖肾煎小剂量组、糖肾煎大剂量组、贝那普利组、中西医结合组,采用单肾切除+高脂饲料+小剂量链脲佐菌素(STZ)腹腔注射+行为干预的方式,建立病证结合的2型糖尿病肾病气阴两虚证大鼠模型。各组分别干预16周后,观察各组大鼠空腹血糖(FBG)、负荷后2h血糖(P2hBG)、空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)、胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白(HDL-c)、低密度脂蛋白(LDL-c),血尿素氮(BUN)、血肌酐(Cr)、肾重/体重指数、24h尿微量白蛋白(UAlb),尿β2微球蛋白(β2MG),免疫组化计算足细胞密度、电镜下足细胞病理改变。结果(1)与假手术组比较,模型组大鼠糖脂代谢异常(P<0.01),尿蛋白排泄率增加(P<0.01),肾功能无明显改变(P>0.05),足细胞密度减小(P<0.01),足突增宽、融合;(2)与模型组比较,糖肾煎小剂量组上述指标均有改善趋势,但无统计学意义(P>0.05),糖肾煎大剂量组、贝那普利组及中西医结合组上述指标均有改善(P<0.05或P<0.01);(3)与贝那普利组比较,糖肾煎大剂量组及中西医结合组HOMA-IR有所改善(P<0.01),余指标无明显变化。结论中药复方糖肾煎对2型糖尿病肾病气阴两虚证大鼠足细胞有保护作用。 Objective To observe the protective effect of Tangshenjian decoction on podocyte of qi and yin deficiency of type 2 diabetic nephropathy rats. Methods Rats were randomly divided into control group, model group, Tangshenjian low dose group, Tangshenjian high dose group, Benazepril group and integrative group. The model of combined disease and syndrome of qi and yin deficiency of type 2 diabetic nephropathy rats. was established by using unilateral nephrectomy+high- fat diet+low dose of streptozotocin by intraperitoneal injection, combined behavioral manner intervention. After 6 weeks of in- tervention, fasting blood glucose (FBG), postprandial 2 hours blood glucose (P2hBG), fasting insulin, insulin resistance (HOMA-IR), cholesterol (TC), triglyceride (TG), high-density lipoprotein (HDL-c), low density lipoprotein (LDL-c), blood urea nitrogen (BUN), serum--creatinine (Cr), kidney weight/body weight, 24h urine trace albumin quantitative(24h-UAlb), urine podocyte under 132 microglobulin (U-132MG) of each group were observed, the podocyte density was calculated, the pathological changes were observed under electron microscope. Results (1) Compared with group N, glucolipids metabolismis abnormal(P〈0.01), urinary albumin excretion rate of group M increased (P〈0.01), renal function showed no significant changes (P〉0.05), podocyte density decreased (P〈0.01), podocyte process was broadening and fusion; (2) Compared with group M, the above indexes were improved in group x, but there was no significant difference (P〉0.05). The indexes of group D, group B and group J improved (P〈0.05 orP〈0.01); (3) Compared with group B, the HOMA-IR in group D and group J was improved (P〈0.01), there was no significant change in the index. Conclusion Tangshenjian decoction has certain protective effect to the podocyt of qi and yin deficiency on type 2 diabetic nephropathy rats.
