高危妊娠孕妇检测优生四项和唐氏筛查对胎儿缺陷早期诊断价值

Clinical value of TORCH and Down's screening in early detection of fetal defect in high-risk pregnant women

目的:探讨在高危妊娠孕妇中检测优生四项(TORCH)和唐氏筛查对胎儿缺陷早期诊断价值。方法:分析2014年3月至2017年3月1 000名孕妇检测TORCH和唐氏筛查结果,其中TORCH检测通过抽取孕妇晨起空腹静脉血,采用酶联免疫方法测定;唐氏筛查通过对孕妇血清中的血浆蛋白A值(AFP)和绒毛膜促性腺激素(free-β-h CG)采用时间分辨荧光免疫法进行双标记测定。并随访追踪胎儿以及孕妇情况。结果:1 000名孕妇经风险筛查后高风险例数22例,占2.20%;低风险例数978例,占97.80%,22例高风险孕妇中不良结局发生率高于978例低风险孕妇(χ2=4.3087,P=0.0379)。结论:高危妊娠孕妇TORCH检测和唐氏筛查是早期判断胎儿异常及预测不良妊娠结局的有效指标,结合染色体核型分析手段,对胎儿缺陷早期诊断价值有重要意义。

Objective：To investigate the value of TORCH and Down＇s screening in the early diagnosis of fetal defects in high - risk pregnant women. Methods： From March 2014 to March 2017, retrospective analysis of 1000 pregnant women before and after the detection of TORCH and Down＇s screening was conducted, in which TORCH detection was determined with pregnant women morning fasting venous blood, using enzyme - linked immunoassay; and Down＇s screening was performed by measuring the serum protein A （AFP） and chorionic ganadotropin （free - β -hCG） in serum via time - resolved fluorescence immunoassay. And there was a follow - up to track the situation of the fetus and pregnant women, using SPSS18.0 for data analysis. Results：Among 1000 cases of pregnant women after risk screening there were 22 high -risk cases, risk ratio account- ing for 2.20 % ; and 978 low - risk cases, risk ratio accounting for 97.80 %. The incidence of adverse outcomes in 22 high - risk pregnant women was significantly higher than that in 978 low - risk pregnant women, the difference statistically significant （ P 〈0.05 ）. Conclusion：TORCH detection and Down＇s screening in high - risk pregnant women is an effective indicator for predicting fetal abnormalities and predicting the outcome of poor pregnancy. Combined with chromosome karyotype analysis, it is of great significance for early diagnosis of fetal defect.
