胃溃疡穿孔修补术联合药物对胃溃疡穿孔的治疗价值探析

Therapeutic Value of Closure of Perforated Gastric Ulcer Combined with Medicine in the Treatment of Perforated Gastric Ulcer

目的分析胃溃疡穿孔修补术联合药物对胃溃疡穿孔的治疗价值。方法回顾性分析2016年2月—2017年1月唐河县人民医院普外科收治的80例胃溃疡穿孔患者的病历资料,根椐治疗方法将其分为观察组(40例)与对照组(40例),其中对照组患者为单纯采用胃溃疡穿孔修补术治疗者,观察组患者为采用胃溃疡穿孔修补术联合药物治疗者,对比分析两组患者的治疗总有效率、SF-36量表评分、住院时间及幽门螺杆菌转阴时间。结果治疗8周后,对照组患者的治疗总有效率为77.5%,观察组患者的治疗总有效率为95.0%,两组对比,χ~2=5.165,P<0.05,差异具有统计学意义;治疗8周后,两组患者SF-36量表评分结果显示,躯体功能及心理功能对比,t'=4.457及3.671,P均<0.01,差异具有统计学意义,社会功能及总分对比,t=6.311及3.972,P均<0.01,差异具有统计学意义;对照组患者住院时间为(6.40±2.71)d,观察组患者住院时间为(4.51±1.24)d,两组对比,t'=4.011,P<0.01,差异具有统计学意义;对照组患者幽门螺杆菌转阴时间为(15.40±3.57)d,观察组患者幽门螺杆菌转阴时间为(10.51±2.61)d,两组对比,t=6.993,P<0.01,差异具有统计学意义。结论胃溃疡穿孔修补术联合药物治疗胃溃疡穿孔,可提高治疗效果,缩短治疗时间,加速幽门螺杆菌转阴,改善患者生活质量,临床应用价值较高,值得推广应用。

Objective To analyze the therapeutic value of closure of perforated gastric ulcer combined with medi- cine in the treatment of perforated gastric ulcer. Methods Retrospective analysis was made to the clinical data of 80 pa- tients with perforated gastric ulcers, admitted to General Surgery Department, Tanghe County People＇ s itospital t＇rom Febru- ary 2016 to January 2017. These patients were divided into observation group （40 cases） and control group （40 cases）. Patients in control group were treated only with closure of perforated gastric ulcer and patients in observation group were trea- ted with closure of perforated gastric ulcer in combination with medicine. Total effective rate, SF-36 modified Rankin scale, hospitalization duration and Helicobacter Pylori negative conversion time of patients in the two groups were analyzed and compared. Results After 8 weeks of treatment, the total effective rate was 77.5% for control group and 95.0% for obser-ration group. The comparison of the two groups showed statistically significant difference （X^2 = 5. 165, P 〈 0. 05）. After 8 weeks of treatment, SF-36 modified Rankin scale of patients in the two groups revealed that the comparison between physical function and psychological function showed statistically significant difference （t＇=4. 457 and 3. 671, P 〈0. 01 ） ; the com- parison between social function and aggregated score showed statistically significant difference （t = 6. 311 and 3. 972, P 〈 0.01 ）. Hospitalization duration was （6.40 ±2. 71 ） days for patients in control group and （4. 51 ±1.24） days for obser- vation group, and the comparison between them showed statistically significant difference （t＇=4.011, P 〈 0. 01 ）. I-IP neg- ative conversion time was （ 15.40 ±3.57 ） days for patients in control group and （ 10. 51 ± 2.61 ） days for observation group, and the comparison between them showed statistically significant difference （ t = 6. 993, P 〈 0. 01 ）. Conclusion Closure of perforated gastric ulcer combined with medicine in the treatment of perforated gastric ulcer can improve curative effect, shorten treatment time, accelerate negative conversion of Helicobacter Pylori and better patients＇ quality of life. It should be promoted in clinical practice due to its good value of clinical application.
